“…Data from the US Registry for FMD suggest that cigarette smoking may be associated with higher prevalence of aneurysm among those with FMD 60 ; however, smoking was not a predictor of the presence of aneurysm in a multivariate analysis of the large European FMD Registry 3 or the ARCADIA-POL study (Assessment of Renal and Cervical Artery Dysplasia-Poland). 59 The initiating events of fibroplasia are unknown and hypothesized to be both genetic and nongenetic, possibly relating to several mechanisms that are unlikely to be assessed in end-stage tissue removed at the time of surgical intervention. These include (1) mechanical stretch of the artery, as the sites of arterial involvement such as the renal and carotid arteries are at locations of stretch with movement or positional change, with vascular injury that would promote phenotypic switching of arterial medial smooth muscle cells (SMCs) to a fibroproliferative phenotype; (2) hormonal milieu, invoked as related to the disproportionate burden of disease in women and the occurrence of some vascular events at times of hormonal changes, such as peripartum and perimenopause; and (3) potential infectious etiologies 61 for which data are limited or other causes that have not yet been considered.…”
