BackgroundDue to the decrease in informal care by family members and the demographic development, the importance of professional geriatric care will rise considerably. Aim of this study was to investigate the psychosocial workplace situation for employees in this profession.MethodsThe German version of the COPSOQ (Copenhagen Psychosocial Questionnaire) was used for the assessment of psychosocial factors at work. The instrument includes 22 scales and 3 single items concerning demands, control, stress, support, and strain.Results between two study groups of geriatric care were compared to each other as well as to employees in general hospital care and a general population mean (COPSOQ database).Statistical analysis included t-tests, ANOVA and multiple comparisons of means. Statistical significance (p < 0.01, two-tailed) and a difference of at least 5 points in mean values were defined as the relevant threshold.ResultsIn total 889 respondents from 36 institutions took part in the study. 412 worked in Home Care (HC), 313 in Geriatric Nursing Homes (GNH), 164 in other professions (e.g. administration).Comparison between HC and GNH showed more favourable values for the first group for the most scales, e.g. lower quantitative and emotional demands and less work-privacy conflict, better possibilities for development etc. Compared to external values from the German COPSOQ database for general hospital care (N = 1.195) and the total mean across all professions, COPSOQ-total (N = 11.168), the results are again positive for HC workers on most of the scales concerning demands and social support. The only negative finding is the very low amount of social relations at work due to the obligation to work alone most of the time. Employees in GNH rate predictability, quality of leadership and feedback higher when compared to general hospital care and show some further favourable mean values compared to the COPSOQ mean value for all professions. A disadvantage for GNH is the high rating for job insecurity.A supplementary subgroup analysis showed that the degree of negative evaluation of psychosocial factors concerning demands was related to the amount of working hours per week and the number of on-call duties.ConclusionsCompared to employees in general hospital care and the COPSOQ overall mean value across all professions, geriatric care employees and especially home care workers evaluate their psychosocial working situation more positive for most aspects. However, this seems partly due to the very high proportion of part-time workers. Critical results for the two study groups are the relatively high job insecurity in nursing homes and the lack of social relations for the HCrs.

Background: With the proportion of the population aged over 65 expecting to rise from 15 to 26% within the next 50 years, the demand for formal support workers for older adults will rise substantially. It is essential that this workforce remain healthy; however, due to the nature of elder care, musculoskeletal complaints are commonplace in support workers. Objectives: The aim of this systematic review was to determine the current work-related musculoskeletal disorders which are present in workers in the aged care sector. Method: Five databases were searched: AMED, CINAHL, EMBASE, Medline, and Web of Science. Four main concepts: aged care, physical health, carer/support worker, and work, were included in the search. All study design types where participants were involved in the formal care of the older adults were included. Results: The search yielded 17 articles after the implementation of strict inclusion and exclusion criteria. Common musculoskeletal complaints included neck, shoulder, and back pain with a prevalence ranging between 5 and 51%. Associated risk factors for injury included: high physical job demands, psychosocial factors, effort-reward imbalance, lack of career opportunities, increased age, and decreased education levels.Conclusion: Current evidence shows that the back, neck, and shoulder are the main musculoskeletal complaints of workers in the aged care sector. However specific diagnoses of these injuries have not been investigated. Further research should focus on quantifying specific disorders, determining risk factors, and developing prevention strategies.

Differences in risk factors for voluntary early retirement and disability pension: a 15-year follow-up in a cohort of nurses’ aides

ObjectiveTo estimate the extent of early retirement and to examine risk factors for voluntary early retirement and disability pension in a cohort of nurses’ aides.DesignRegister study including baseline questionnaire and register data covering all transfer incomes from 1991 to 2008 in a cohort of nurses’ aides established in 1993 with a follow-up period of 15 years.SettingNurses’ aides working in nursery homes, homecare or hospitals.Participants3332 gainfully employed nurses’ aides at the time of inclusion in the study.OutcomeDisability pension or early voluntary retirement.Results16.2% of the population was granted disability pension and 27.1% entered early voluntary retirement in the follow-up period representing 11 186 lost working years with a direct cost in transfer payment amounting about €410 million. Health-related risk factors for disability pension was long-lasting low-back pain (HR 2.27(95% CI 1.55 to 3.34), sick leave because of upper-extremity disorders (HR 2.18 (95% CI 1.08 to 2.11), and inflammatory rheumatic disease (HR 2.42 (95% CI 1.67 to 3.52)). Of non-health-related factors, low education, workers compensation case, evening work and high-rated perceived exertion at work all were minor risk factors for disability pension. The primary risk factor for early voluntary retirement was low education (HR 3.19 (95% CI 2.65 to 3.85)).Conclusions43.3% of nurses’ aides gainfully employed in 1993 retired before due time during the follow-up period. Work-related factors at baseline only seemed to have a minor prognostic role. Risk factors for disability pension were mainly health-related factors, whereas economical factors seemed to influence the decision to choose early voluntary retirement. The number of persons and the amount of lost working years underscores the need of a more active counselling towards maintaining employment especially among those with persistent musculoskeletal disorders.
