Introduction:Lichen planus is a chronic inflammatory disorder in which a delayed type of hypersensitivity immune reaction plays a major role in the pathogenesis. The disease and mainly affects skin, mucous membranes, nails and hair follicles. Objective: To study cardiovascular risk factors (metabolic syndrome and lipid profile) in patients of lichen planus. Material and methods: Fifty patients of lichen planus aged 18 years or above and fifty age-and sex-matched controls without any evidence of skin disorder including lichen planus were enrolled. Atherosclerotic risk factors were evaluated as per National Cholesterol Education Project Adult Treatment Panel III (NCEP-ATPIII). Metabolic syndrome was defined as per the definition of the South Asian Modified criteria of NCEP-ATPIII11. Results: The mean total cholesterol was 179.2 ±29.74 and 164.74 ±22.18 mg/dl in lichen planus patients and controls, respectively, and the difference was found to be significant (p = 0.006). A significant difference between patients and controls was also found for low-density lipoprotein (p = 0.001). Although all atherosclerotic factors were elevated in patients compared to controls, the difference was statistically significant only for body mass index (p = 0.003989). Conclusions: The present study found a higher prevalence of dyslipidemia with a significant association between lichen planus and higher total cholesterol and low-density lipoprotein lipids. Body mass index, blood pressure and blood glucose were also higher in patients with lichen planus.

Background: Metabolic syndrome (MetS), insulin resistance (IR) and papulosquamous diseases with shared pathophysiology leads to conglomeration of risk factors of non-communicable diseases like cardiovascular disease (CVD) and type 2 diabetes (T2D) causing high mortality worldwide. The objective is to study the association of MetS and IR in papulosquamous diseases. Methods: It is a hospital-based case-control study conducted on 200 age-sex matched cases and controls with consent. Subjects were administered a pre-validated questionnaire, diagnosis of papulosquamous diseases was based on clinical examination and skin biopsy. MetS was diagnosed based on national cholesterol education program's- adult treatment plan iii with Asian modification for abdominal circumference criteria. Fasting serum insulin levels (FI) was used as a surrogate marker of IR. Results: Mean serum HDL- C level was low and statistically significant (p=0.017). Mean fasting plasma glucose level was significantly higher in patients (p=0.008). Mean FI level was significant between cases and controls (p=0.013). IR was found in 76% of cases which was significantly higher than in controls (p<0.05). IR was highest in psoriasis 29% followed by LP 22%, PRP 19%, and LS 6% which was statistically significant (p<0.05). MetS was mostly found in psoriasis (17%), LP (9%) and least in PRP, LS. Conclusions: This study intends clinicians to do periodic MetS and IR evaluation in papulosquamous diseases. Thus, subclinical cases of non-communicable diseases can be detected and potential co-morbidities can be prevented.

The prevalence of LP is almost 0.89% of the population, and it commonly affects middle-aged individuals of both genders. The pathogenesis and aetiology of the disease are not completely known. 1 LP is a T cell-mediated auto-inflammatory disease. In pathogenesis, T cell activation acts a significant role. Activated T-cells induce the T helper type 1 (Th1) response. 2 An inflammatory response caused by Th1 cytokines (IL-2, 4, 6, 10) and TNFα also acts a role in the pathogenesis of metabolic syndrome. 3 Dyslipidaemia, which is one of the metabolic syndrome components, has been found to be associated with LP. Chronic inflammation is the main factor in its relationship with dyslipidaemia in patients with LP. 3 The relationship of chronic inflammatory skin diseases with MS has been more clearly demonstrated in recent years. 3 Again, in this context, the number of publications showing the relationship with MS in lichen planus patients has increased. 1,4 Acanthosis nigricans is a clinical finding described by asymptomatic plaques that are grayish-brown or black in color, raised from the skin, lichenified, velvety, and violase, with no sharp borders, located in intertriginous areas such as axilla, neck, and groin. 5 Hyperkeratosis and papillomatosis are prominent in histopathological examination. 5
