Prevalence of malnutrition in a cohort of 509 patients with acute hip fracture: the importance of a comprehensive assessment

Abstract: Protein malnutrition and vitamin D deficiency are the rule in acute HF patients. Energy malnutrition and sarcopenia are also common. A nutritional assessment in these patients should include these aspects together.

“…Over the last few years, cystatin C has been proposed as an alternative to creatinine for GFR estimate, since it is not influenced by muscle mass and nutritional status, which are known to be the main determinants of creatinine serum levels [34]. Therefore, cystatin C is expected to provide a more reliable estimate of GFR than creatinine in patients with sarcopenia and malnutrition [13], which are typical features of older fracture patients [12,35]. Thus, non-cystatin based equations may overestimate renal function; conversely, cystatin based equations are able to identify patients with a moderate renal impairment that is not detected by other formulas and that, differently from other estimates, is independently associated with delirium.…”

“…Older hip fracture patients are often malnourished or at risk of malnutrition [19][20][21]. Malnutrition can be caused by multiple factors including a reduced dietary intake (due to a lack of appetite, inability to eat or oral health problems), malabsorption, increased nutrient losses or altered metabolic requirements [22].…”

“…Similarly, other researchers found lower s-alb levels in the hip fracture patients compared to controls 22 . Furthermore, as shown in other studies, low s-alb levels are associated with morbidity and mortality in hip fracture patients 23 .…”
