Prevalence of Gastric and Duodenal Polyps and Risk Factors for Duodenal Neoplasm in Korean Patients with Familial Adenomatous Polyposis

Abstract: Background/Aims: The prevalence of gastric polyps, duodenal adenoma and duodenal cancer has been reported as being high among familial adenomatous polyposis (FAP) patients, but there have been no reports of this association in Korea. This study evaluated the prevalence of gastric and duodenal polyps and risk factors for duodenal neoplasm in FAP patients in Korea. Methods: We reviewed both initial and follow-up endoscopic results from FAP patients. We also investigated the treatment modality of duodenal adenoma… Show more

“…Fundic gland polyps (FGPs) are the most common gastric tumours/polyps in FAP patients, and the frequency of FGPs ranges from 20 to 88% in FAP patients (9)(10)(11). Gastric adenoma is also common in FAP patients and develops in 9-50% of these patients (12)(13)(14)(15). Gastric cancer is more frequently found in Asian FAP patients than in Western FAP patients, which is similar to the corresponding rates in the general population.…”

“…However, environmental and hereditary factors may also affect cancer incidence in FAP cohorts. For example, when compared to Western countries, the incidence of gastric and duodenal cancers has been reported to be higher and lower in Asia, respectively (18,34). Furthermore, length of follow-up, endoscopic techniques (use of side-viewing endoscopy, high resolution endoscopy, chromoendoscopy), strategies for prevention, frequent endoscopic resection of at risk lesions, ampullary localization have also been incriminated to influence incidence rates (23).…”

“…Local management of duodenal polyposis is associated with high rates of recurrence, although it may postpone cancer development. Thus, endoscopic or surgical management of patients diagnosed with large polyps, high-grade dysplasia or severe duodenal polyposis turned to be important preventive tools to decrease cancer deaths in FAP patients (1,18). Otherwise, patients diagnosed with duodenal cancer should undergo radical pancreaticoduodenectomy that may carry substantial morbidity and mortality rates (19,20).…”

“…no hubo casos de cáncer gástrico en el período evaluado. En series occidentales no se reporta una mayor incidencia de cáncer gástrico cuando se compara con la población general, mientras que en series de Japón y Corea 16 , se reporta una mayor incidencia de cáncer gástrico en los pacientes con PAF. Por lo anterior el seguimiento del tracto digestivo está marcado por la enfermedad duodenal más que por el desarrollo de pólipos gástricos.…”
