Despite advances in scientific research, Staphylococccus aureus remains a pyogenic and toxigenic bacterium involved in different infections, it endowed with the capacity to infect several biotopes and cause a wide range of infections ranging from skin diseases to other serious pathologies such as pneumonia, meningitis, sepsis, osteomyelitis and infectious endocarditis. Moreover, the emergence of resistant strains constitutes a serious public health problem. Thus, the development of new active compounds from natural sources such as medicinal plants is urgently needed. To this end, the aim of our review was to describe the state of art of infections caused by S. aureus, its pathogenesis, treatment and to provide a synthesis about studies reporting a bio guided isolation of most promising compounds selected for their anti-staphylococcal activity.

Many studies have been published assessing the association between the presence of S. aureus genes and outcomes in patients with bone and joint infections (BJI), but it is not known if they have had similar findings. A systematic literature review was performed. All available data on studies in Pubmed between January 2000 to October 2022 reporting the genetic characteristics of S. aureus and the outcomes of BJIs were analyzed. BJI included prosthetic joint infection (PJI), osteomyelitis (OM), diabetic foot infection (DFI), and septic arthritis. Because of the heterogeneity of studies and outcomes, no meta-analysis was performed. With the search strategy, 34 articles were included: 15 articles on children and 19 articles on adults. In children, most BJI studied were OM (n = 13) and septic arthritis (n = 9). Panton Valentine leucocidin (PVL) genes were associated with higher biological inflammatory markers at presentation (n = 4 studies), more febrile days (n = 3), and more complicated/severe infection (n = 4). Other genes were reported anecdotally associated with poor outcomes. In adults, six studies reported outcomes in patients with PJI, 2 with DFI, 3 with OM, and 3 with various BJI. Several genes were associated with a variety of poor outcomes in adults, but studies found contradictory results. Whereas PVL genes were associated with poor outcomes in children, no specific genes were reported similarly in adults. Additional studies with homogenous BJI and larger sample sizes are needed.

Pastaraisiaismetais daugėja pranešimų apie tai, kad sergamumas sunkiomis ir komplikuotomis visuomenėje įgytomis vaikų pneumonijomis (VĮVP) įvairiose šalyse didėja, nors sergamumas pneumonija sumažėjo. Nepaisant skiepijimo pneumokokiniais skiepais, Streptococcus pneumoniae yra dažniausias bakterinės pneumonijos sukėlėjas, ypač – jo serotipai, neįeinantys į pneumokokinius skiepus. VĮVP komplikacijas galima skirstyti į plautines ir sistemines. Dažniausios jų – parapneumoninis pleuritas, pleuros empiema, abscesas, nekrozinė pneumonija, taip pat sepsis ir hemolizinis ureminis sindromas. Komplikuotą VĮVP reikėtų įtarti, kai būklė staigiai blogėja arba nėra teigiamo atsako per 48 val. po skirto adekvataus gydymo bei progresuoja kvėpavimo nepakankamumas. Plaučių rentgenograma bei ultragarsinis tyrimas – tai dažniausi ir paprasčiausi tyrimo metodai komplikuotų pneumonijų diagnostikai, tačiau apie 20 proc. atvejų pokyčiai gali būti nematomi. Tokiu atveju, kai yra indikacijų, rekomenduojama atlikti krūtinės ląstos kompiuterinę tomografiją. Visi vaikai, sergantys sunkia komplikuota pneumonija, turi būti gydomi III lygio arba jam prilygstančiame stacionare, kur yra vaikų intensyviosios terapijos skyrius, galimybė nuodugniam tyrimui ir daugiadalykei priežiūrai.
