Background: To examine time trends in comorbidity of hypertension and type 2 diabetes (T2DM) and their diagnosis, treatment, and management in China during 2000-2015 and study factors associated with these outcomes. Methods: Longitudinal data collected from the China Health and Nutrition Survey (CHNS) during 2000-2015 were analyzed. Of the 28,204 adults, 832 had both hypertension and T2DM were selected. Average systolic blood pressure (SBP) and diastolic blood pressure (DBP) and hypertension prevalence among T2DM patients, and treatment and control of hypertension and T2DM among patients with both conditions were examined for all and by sex and weight status. Mixed effects models assessed the associations. Results: From 2000 to 2015, among patients with T2DM, hypertension prevalence dropped from 88.4% to 83.0% and BPs decreased (P < 0.05). Men and overweight/obese patients had greater decreases in hypertension prevalence and DBP, while women had a larger decrease in SBP than men. Over time, among patients with both hypertension and T2DM, rates of hypertension treatment (45.3% to 57.7%), hypertension control (3.0% to 10.9%) and T2DM treatment (90.0% to 95.6%) increased, but were still very low except for T2DM treatment (all P < 0.001). Age, gender, smoking, drinking, household income, education, residence, and weight status were significantly associated with prevalence, treatment, and control of hypertension, and T2DM treatment among patients with both hypertension and T2DM. Conclusion: Rates of hypertension treatment and control among patients with both hypertension and T2DM have improved in recent years, but were still low.

Summary With the high prevalence of diabetes, its long‐term treatment, and many complications, there is a need for a new diabetes self‐management model in developing countries. Therefore, this study aimed to assess the implementation and effect of a new diabetes self‐management model, known as the healthcare cloud information platform, in Shanghai, China. The implementation of the new model was evaluated in terms of the equipment coverage in community health service centres and the services provided. The effect was assessed according to the treatment rate and control rate from 2012 to 2017. Their correlations with the self‐management rate were analysed using Spearman's test. It was found that the Shanghai healthcare cloud information platform had covered a total of 237 community health service centres in all of the 16 administrative districts, equipping them with 2489 health stations, by 2017. An average of 8966 people uploaded data to the healthcare cloud information platform every day, with the accumulated service up to 12 043 508 person‐times and an average daily increase of 13 227.9. The treatment rate of diabetes patients in Shanghai increased from 42.9% in 2012 to 78.0% in 2017, and the control rate increased to 72% in 2017, compared with 63% in 2012. Furthermore, there was a significant correlation between the treatment rate, control rate, and self‐management rate. The findings indicate that it is feasible to implement the new healthcare cloud information platform for diabetes self‐management and that it is effective.

ObjectiveTo describe the implementation, main intervention areas and initial results of the Integrated Strategy for the Promotion of Healthy Eating (EIPAS) in Portugal.MethodsEIPAS was published as a Law, in December of 2017, as a result of a collaboration between several ministries, including the Finance, Internal Affairs, Education, Health, Economy, Agriculture, and Sea Ministries, aiming at improving the dietary habits of the Portuguese population. The working group, led by the Ministry of Health, developed this strategy for over a year. The framework produced was based on WHO and European Commission recommendations as well as on relevant data from the last Portuguese dietary intake survey (2015/2016). EIPAS also reflects the results of a public hearing, including the food industry, among others, and the experience gathered, since 2012, through the National Programme for the Promotion of Healthy Eating. It considers the ‘health in all policies’ challenge set by WHO and has four different strategic areas, namely (1) creation of healthier food environments, (2) improvement of the quality and accessibility of healthy food choices for consumers, (3) promotion and development of literacy, in order to encourage healthy food choices, and (4) promotion of innovation and entrepreneurship. In order to achieve these goals, a set of 51 actions was established and assigned to the seven ministries involved.ResultsUnder the scope of this strategy, Portugal has already implemented several actions, including (1) definition of standards for food availability at all public healthcare institutions; (2) implementation of a sugar tax on sweetened beverages; (3) implementation of a voluntary agreement with the food industry sector for food reformulation (work in progress); (4) design of a proposal for an interpretative model of front-of-pack food labelling; (5) improvement of the nutritional quality of food aid programmes for low-income groups; and (6) regulation of marketing of unhealthy foods to children.ConclusionsFor the first time, Portugal has a nutrition policy based on the WHO concept of ‘health in all policies’ and on the national data on food intake. The implementing process of all 51 actions and the inherent complexities and difficulties found so far have made this process be an authentic political and social laboratory that deserves to be followed.
