SUMMARYObjective: The aim of this study was to determine the knowledge and awareness of female sex workers (FSWs) about human papillomavirus (HPV) infection in Turkey.Methods: 239 brothel-based FSWs were recruited for an interview. A questionnaire was completed by face to face interview. The demographic features, the level of knowledge, and awareness about HPV infection of the participants were recorded.Results: A total of 152 (63.6%) were over 30 years of age, and 210 (87.9%) completed primary education. 122 FSWs (50.9%) were using condom, and about one third had high frequency of sexual contacts. Knowledge scores were ≤4 in 181 (75.7%) FSWs. Low education and higher age group were not significant predictors of low knowledge scores. Low education was not an independent risk factor for awareness but higher age group participants were significantly less aware of risks associated with HPV infection (p = 0.019) although overall score was very poor (0.60 ± 0.68).Conclusions: The knowledge and awareness about HPV infection and its possible consequences (cervical cancer) as well as the utility of screening methods among FSWs in Turkey are extremely poor. HPV-focused educational programmes for targeted populations, e.g. FSWs and young adults prior to their sexual activity, should be launched urgently to increase awareness of the risks associated with HPV infection and thus reducing the incidence of cervical cancer in Turkey in the future.

Barriers to Cervical Screening Among Sex Workers in Vancouver

Objectives We longitudinally examined the social, structural, and geographic correlates of cervical screening among sex workers in Metropolitan Vancouver, British Columbia, to determine the roles that physical and social geography play in routine reproductive health care access. Methods Analysis drew on (2010–2013) data from an open prospective cohort of sex workers (An Evaluation of Sex Workers’ Health Access). We used multivariable logistic regression with generalized estimating equations (GEE) to model correlates of regular cervical screening. Results At baseline, 236 (38.6%) of 611 sex workers in our sample had received cervical screening, and 63 (10.3%) were HIV-seropositive. In multivariate GEE analysis, HIV seropositivity (adjusted odds ratio [AOR]=1.65; 95% confidence interval [CI]=1.06, 2.58) and accessing outreach services (AOR=1.35; 95% CI=1.09, 1.66) were correlated with regular cervical screening. Experiencing barriers to health care access (e.g., poor treatment by health care staff, limited hours of operation, and language barriers) reduced odds of regular Papanicolaou testing (AOR = 0.81; 95% CI = 0.65, 1.00). Conclusions Sex workers in Metropolitan Vancouver had suboptimal levels of cervical screening. Innovative mobile outreach service delivery models offering cervical screening as one component of sex worker–targeted comprehensive sexual and reproductive health services may hold promise.

Abstract: HR-HPV prevalence is very high in FSW and varies by geographic origin. Women of younger age and lower education level and those engaged in sex work for <1 year showed the highest prevalence, making them a priority group for cervical cancer prevention programs.
