Prevalence and distribution of plasmid-mediated quinolone resistance genes in clinical isolates of Escherichia coli lacking extended-spectrum  -lactamases

Abstract: In the UK, aac(6')-Ib-cr occurs in E. coli in the absence of CTX-M-15, but with a restricted serotype distribution. Its presence in widespread bacteraemia isolates of a single type from 2001 to 2002, prior to the spread of CTX-M-15 in Britain, might suggest a lineage from which plasmid recombination occurred in man or other species.

“…ST393 with CTX-M-14 and -15 had mostly been described in urinary isolates from Spain (1) and South Korea (18). ST405 with various types of CTX-Ms has a worldwide distribution (7,14,19,35), while ST648 with CTX-M-15 and -32 is present in poultry from Spain (8) and humans in China (44) and the Netherlands (41). ST405 and ST648 are also associated with NDM ␤-lactamases (11,21).…”

“…Two narrow-spectrum ␤-lactamases, OXA-1 and TEM-1, have frequently been found in ST131 isolates, generally in association with CTX-M-15 enzymes, following plasmid transfer (see "Characterization of plasmids harbored by clinical isolates of E. coli ST131" above) (9,55,58,62,94,106). Another resistance gene, aac(6=)-Ibcr, which confers resistance to both aminoglycosides (amikacin and tobramycin) and ciprofloxacin, has frequently been detected in association with CTX-M-15 enzymes (9,19,(107)(108)(109)(110). Only a few studies in Spain and Portugal have reported the detection of qnr determinants, another plasmid-mediated mechanism of quinolone resistance in ST131 isolates (111)(112)(113).…”

“…PCR conditions were: 5 min of denaturation at 95°C; 30 cycles of 95°C for 30 s, 57°C for 30 s, and 72°C for 50 s; and then 10 min of elongation at 72°C. All aac(6Ј)-Ib-positive isolates underwent digestion of the PCR product with FokI and NdeI (New England Biolabs, Ipswich, MA) to identify the aac(6Ј)-Ib-cr variant, which encodes a bifunctional aminoglycoside-fluoroquinolone modifying enzyme (27).…”
