Prevalence and distribution of HHV-8 in different subpopulations, with and without HIV infection, in Spain

Abstract: The HHV-8 prevalence among blood donors in Spain is higher than in Northern Europe and the USA, but is similar to that in Northern Italy. The distribution of HHV-8 is compatible with a sexually transmitted agent. The distribution of HHV-8 correlates with that of Kaposi's sarcoma but factors other than HHV-8 seem to explain the Kaposi sarcoma distribution.

“…A bias in our estimates could take place if the underlying HHV-8 prevalence of the control population could affect hospitalisation rates due to HHV-8-related diseases. Available data from the Spanish general population, however, indicate that the HHV-8 prevalence ranges between 6 and 8% (Gambus et al, 2001;de Sanjose et al, 2002), which is similar to the 5.3% observed in our control subjects, and also conforms to the moderate prevalence observed in Mediterranean countries. Owing to small numbers in some lymphoma categories, we cannot exclude that some of the results are due to chance.…”

“…Slides were then washed in blocking buffer, and fluorescein isothiocyanateconjugated rabbit anti-human immunoglobulin G (IgG) antibody (P0214; DAKO A/S, Denmark), diluted at 1:60 in the blocking solution, was added. Following incubation at 22°C for 22 min, slides were washed in PBS and examined under a fluorescence microscope for nuclear punctate staining (15).…”

“…In the absence of a culture system derived from KS tumorassociated cell lines to allow direct demonstration of KSHV (2,18), the diagnosis of KSHV infection has relied on molecular techniques and the use of serological assays to detect antibodies to KSHV antigens produced during latent or lytic phases of the viral life cycle (13,15,26). The immunofluorescence assay to detect anti-LANA antibodies (IFA-LANA) has long been considered a reference serological test to detect KSHV infection (23).…”

“…However, hot spots also occur in low-incidence regions: in the Po Valley in northern (mainland) Italy, the incidence is 2.5/100,000 men and 0.7/100,000 women, with one rural zone having double the male rate and four times the female rate (24). Elsewhere in Mediterranean Europe, a group of blood donors in Spain had a low KSHV seropositivity similar to that of northern Italians (171). The high prevalence of KSHV in the Mediterranean contrasts with non-Mediterranean European countries such as Latvia, where a recent screening of 150 healthy blood donors by PCR detected human cytomegalovirus, HHV-6, and HHV-7 but failed to detect KSHV (269).…”
