Preterm birth is strongly affected by the glucocorticoid dose during pregnancy in women complicated by systemic lupus erythematosus

Abstract: Background This study aimed to investigate the effect of glucocorticoid doses on adverse pregnancy outcomes (APOs) in women complicated by systemic lupus erythematosus (SLE). Methods We investigated 74 pregnancies complicated by SLE or SLE-dominant mixed connective tissue disease. The pregnancies were managed from conception to delivery in our institution. We retrospectively evaluated whether the mean glucocorticoid dose during pregnancy is associa… Show more

“…We reported that in pregnancies complicated with SLE, the cutoff dosage of glucocorticoids affecting preterm birth is as low as 10 mg/day. [24] In this study, preterm birth occurred in 2 pregnancies with glucocorticoid treatment at a mean dosage of <20 mg/day. These results suggest that even a glucocorticoid dosage of below 20 mg/day is associated with preterm birth.…”

“…15 Previous studies have shown that a high disease activity, antiphospholipid antibodies, lupus nephritis, and the glucocorticoid dose are risk factors for these APOs. 616 We have also shown that disease activity parameters and glucocorticoids were associated with APOs and the Apgar scores of newborns. 11,16 Thus, uncontrolled disease activity and its parameters, such as complement or anti-double-stranded DNA (anti-dsDNA) antibodies, are strongly related to APOs, and the strict control of SLE disease activity in women with SLE who hope to become pregnant is required.…”
