Prestroke Disability and Outcome After Thrombectomy for Emergent Anterior Circulation Large Vessel Occlusion Stroke

Abstract: ObjectiveTo determine the impact of endovascular therapy for large vessel occlusion stroke in patients with pre-morbid disability versus those without.MethodsWe performed a post-hoc analysis of the TREVO Stent-Retriever Acute Stroke (TRACK) Registry, which collected data on 634 consecutive stroke patients treated with the Trevo device as first-line EVT at 23 centers in the United States. We included patients with internal carotid or middle cerebral (M1/M2 segment) artery occlusions and the study exposure was p… Show more

“…We applaud the TRACK Registry investigators for their evaluation of outcomes after thrombectomy in patients with prestroke disability. 1 Although we strongly agree with the investigators that patients with preexisting disability will benefit more from endovascular therapy than they otherwise would from medical management, we are curious as to why the investigators selected the primary outcome of accumulated disability. For a condition in which patients are more likely to have long-term disability (modified Rankin Score >2) than to regain functional independence-despite thrombectomy-assessing for zero functional decline introduces an outcome misclassification bias that preferentially favors patients with prestroke disability.…”

“…We read with interest the article by de Havenon et al 1 In this study, the authors performed a post hoc analysis in patients with thrombectomy to examine the relationship between premorbid disability and outcome of endovascular therapy by using the modified Rankin Scale (mRS). They found that patients without premorbid disability were more likely to accumulate further disability than those patients with premorbid disability, but patients with premorbid disability had a higher mortality.…”

“…A total of 14 articles from 13 author groups were included. Ten studies compared outcomes between patients with versus without pre-morbid stroke disability treated with EVT 14–24. We found two studies comparing patients with pre-morbid disability treated with EVT to pre-morbidly disabled patients receiving medical management alone 25 26.…”

“…Future RCTs should pay closer attention to these details so that findings can be more relatable between studies and increase eligibility. Fewer studies have investigated the clinical outcome of EVT in PSD patients with an mRS score ≥ 2 ( 25 28 ). Their conclusions were consistent and in agreement with the findings of the present meta-analysis.…”
