SUMMARY The influence of papaverine on the intracranial system of the dog was studied by measuring the pressure-depth-time response for the intact intracranial system, i.e., for the subarachnoid and subpial compartments. This was accomplished by a measurement system which provided an accurate pressure-depth determination and a uniform rate of transducer insertion. Long-term Meesuring of Ventricular Fluid Pressure in Spaceoccupying Intracranial Processes and Acute Brain LesionsBy a simplification of the LundbBrg-method it was increasingly possible to make use of a ventricular fluid pressure monitoring for a peri-operative treatment of space-occupying lesions.The present survey is based upon VFP-measurings of 236 patients. The peri-operative dynamics of the mean ventricular fluid pressure (VFPx) and of the meximum ventricular fluid pressure (VFPmx) during the period of a day is examined. Typical, atypical, spontaneaus and provoked alterations of VFP are differentiated.The relation between intracranial pressure and neurological as weil as vegetative symptoms, specifically the so-called brain pressure symptomatology, are analyzed.During treatment VFP-dynamics essentially depends on the kind, the localisation and the operability of the processes.As for the prognosis it showed that in general increases of VFPx above 40 mmHg are distinctly recognizable risk factors. When this is simultaneaus to a severe disturbanca of brain function or of brain metabolism, then increases of VFPx above 20 mmHg are Iife-threatening.A consequenca of this is that in a severe neurological disturbance, for example in all comatose patients, the peri-operative treatment must aim to a prompt normalisation of the intracranial pressure. Zusammenfassung Die Technik der Langzeitmessung des Ventrikelliquordruckes (VLD) konnte seit ihrer Einführung durch Lundberg (1960) weiter vereinfacht und für die peri-operative Behandlung der raumfordernden intrakraniellen Prozesse zunehmend genutzt werden. Die vorliegende Untersuchung stützt sich auf Langzeitmessungen des Ventrikelliquordruckes bei 236 Patienten mit raumfordernden intrakraniellen Prozessen. Das peri-operative Verhalten des mittleren Ventrikelliquordruckes (VLDx) und der maximalen Druckanstiege innerhalb einer Tagesperiode (VLDmx) wird baschrieben. Dabei werden typische und atypische, spontane und induzierte Schwankungen des VLD unterschieden. Die Beziehung des Ventrikelliquordruckes zur neurologischen Symptomatik und zu den vegetativen Funktionen, insbesondere zur sog. Hirndrucksymptomatik, werden untersucht. Das Verhalten des Ventrikelliquordruckes im Behandlungsverlauf hängt wesentlich ab von Art, Lokalisation und Operabilität der Prozesse. Hinsichtlich des Behandlungsverlaufes, d.h. der Prognose, er· gab sich, daß allgemein Anstiege des VLDx über 40 mmHg, blei gleichzeitigem Vorliegen einer schweren neurologischen oder metabolischen Störung Steigerungen des VLDx über 20 mmHg bereits einen verlaufsbestimmenden zusätzlichen Risikofaktor darstellen. Daraus ergibt sich die Konsequenz, daß bei einer schweren Hirnfunktionsstörung die peri-operative Therapie raumfordernder intrakranieller Prozesse vor allem auch auf eine rasche Normal isierung des intrakraniellen Druckes gerichtet sein muß.Die während der letzten Jahre in die Klinik eingeführte kontinuierliche Messung des Schädelinnendruckes hat das bisherige Wissen von Pathogenese, Symptomatologie und Therapi...
