Pressure injury identification, measurement, coding, and reporting: Key challenges and opportunities

Abstract: Pressure injury (PI) rates are a commonly used indicator of performance of health care facilities, both in acute and subacute settings. However, measuring PI rates in an accurate and reproducible fashion has been challenging. The consequences of poor measurement may include failure to identify poorly performing institutions or incorrect accusations of poor quality care. In this article, we describe the main challenges in identification, coding, and reporting of PIs. Issues include inconsistent identification o… Show more

“…There is a lack of national benchmark data available for comparison. 47 Data were collected within the first hour of presentation to the ED; therefore, any subsequent PI interventions implemented have not been accounted for. Also, participants were recruited on weekdays only and may not be a true reflection of weekly prevalence.…”

“…In addition to health care‐related costs, PIs have a considerable impact on individuals, including pain, odour of exudate, higher susceptibility to infection and poorer health outcomes, and reduced quality of life . The incidence of PIs is considered an indicator of overall health service quality …”

“…16 The incidence of PIs is considered an indicator of overall health service quality. [17][18][19][20] Effective prevention of PIs involves the implementation of PI risk assessment tools, procedures for alleviating pressure on areas that are at risk of tissue breakdown, regular repositioning, appropriate skin care, and optimal nutrition and hydration. 1 Although most PIs in the general population are considered preventable when correct prevention strategies are implemented, 21 PIs in palliative care patients may be unavoidable as a result of skin failure.…”

“…The main factors in the occurrence of injury are stress and tissue tolerance. Stress factors include compressive strength and duration; tissue tolerance is usually affected by the patient's patient's condition and the external microenvironment (Tirgari et al, 2018;Weller et al, 2018). Since the formation of stress injuries can be avoided, prevention is the main task in the clinic.…”
