Preservative-Free Triamcinolone Acetonide Suspension Developed for Intravitreal Injection

Abstract: We developed a simple, reproducible method to compound stable, completely preservative-free TACA suspensions with a reasonable shelf-life, which enables to study the effect of intravitreal TACA--not biased by varying doses and toxic compounds or their residues.

“…6 For intravitreal TA application, possible retinal toxicity from the preservative benzyl alcohol has been a concern, which stimulated the various decanting techniques 35 and the development of preservative-free TA formulations (Triesence; Alcon Laboratories, Inc.; Trivaris, Allergan, Inc). Thus far, toxicity studies of commercially preserved intravitreal TA have been performed in various settings and the results have been conflicting.…”

“…The evidence of retinal toxicity secondary to the compounds in preserved TA formulations such as benzyl alcohol led to an urgent need to find alternative compounds and to create preservative-free formulations. In 2006, Bitter and colleagues developed a simple, reproducible method to produce stable, preservative-free TA (PF-TA) suspensions that can be successfully applied intravitreally [20]. Subsequently, Kim et al evaluated the safety and pharmacokinetics of the preservative-free formulation of TA in intravitreal injections in rabbits.…”

“…In addition, several complications have been related to this route of administration, such as intraocular infection, retinal detachment, and lens injury [19]. Preservative-containing TA formulations have been shown to produce retinal toxicity after their administration, leading to the development of preservative-free TA formulations [20]. Antiangiogenic drugs have displaced the use of steroids for vitreoretinal diseases and have become the gold standard treatment for these pathologies [21].…”

“…[5][6][7]27 Triamcinolone acetonide was prepared preservative-free through a sedimentation technique previously described. 28,29 Each patient gave writ-ten informed consent after the experimental nature, risks, and benefits of all of the therapy options had been explained. The study followed the guidelines of the Declaration of Helsinki.…”
