Preseptal transconjunctival approach for orbital floor fracture repair: ophthalmologic results in 209 patients

Abstract: The commonest cause of injury was forms of violence (32%). An isolated fracture of the orbital floor had occurred in 62 cases, while concomitant facial fractures were present in the remaining 147. A total of 24 patients (11%) had an exophthalmos and 13 (9%) an enophthalmos. Most patients (69%) complained of infraorbital dysesthesia. When elevating the eyeball, the majority of patients showed a marked or severe disturbance of ocular motility (53% in abduction; 51% in adduction) and diplopia (37% in abduction; 3… Show more

“…BP grafts have been extensively used in human patients in different pathologic conditions including arterial or venous dissection, tracheal rupture, cardiac defects such as tetralogy of Fallot or atrial or ventricular septal defect, diaphragmatic resection, and abdominal wall defects . In the field of ophthalmology, BP patch transplantation has been described for the management of neurotrophic corneal ulcers, corneal perforation, corneo‐scleral reconstruction after tumor resection, orbital floor reconstruction, and to cover glaucoma drainage devices or hydroxyapatite orbital implants …”

“…20 In ophthalmology, it has been used for orbital fracture repair, in cases in which the floor fracture can be repositioned to obtain an orbital floor with a smooth surface. 21 The wrap of hydroxyapatite implants with Tutopatch has been suggested as a safe alternative in patients who undergo enucleation for uveal melanoma, with a low rate of complications and a decreased surgical time compared to the wrap with autologous tissue that needs to be harvested. 22 In glaucoma surgery, a double layer of Tutopatch has been suggested to be useful for the implantation of an Ahmed valve in a patient with anterior necrotizing scleritis.…”

“…We used the preseptal approach preferentially as it provides an anatomical plane of dissection without the need to expose the periorbital fat (Schmal et al, 2006).…”
