Presentation of Hodgkin's disease as an endobronchial lesion

Abstract: Presentation of Hodgkin's disease as an endobronchial lesion is rare. Four such cases are described and nine other previously reported cases are reviewed. The difficulties in diagnosis and management, when the disease presents in this manner, are discussed. This diagnosis must be considered when patients present with an endobronchial tumor because careful staging and treatment may lead to cure with the avoidance of major surgery. “…Enlarged hilar lymph nodes were present radiologically in 21 cases (but chest X-ray appeared to be normal in three cases). The lesion was bronchoscopically described under the form of plaquelike infiltrates of the mucosa with superficial ulceration, or a single polypoid or adenomatous mass projecting into the lumen [12]. In 17 patients, the diagnosis was made by bronchoscopic biopsy.…”

“…Based on autopsy data, the incidence of intrabronchial involvement in cases of established Hodgkin's disease may be as high as 15%, but most frequently develops during the disease. Initial endobronchial involvement is very rare; only 34 cases, which fulfilled our inclusion criteria, were found in the literature [9][10][11][12][13][14][15][16]. However, many of these were published a long time ago, and precise data are lacking.…”

“…The lesions can be either plaque-like infiltrates with superficial mucosal ulceration or nodular masses that partially or totally occlude the lumen. Multiple lesions are usual and single polypoidal or adenomatous masses are rare [1,3]. We present two cases that presented initially as endobronchial HD and briefly review the literature.…”

“…Intrathoracic involvement of the lymph nodes, lung parenchyma, and tracheobronchial tree in Hodgkin's disease (HD) occurs in up to 40% of the cases [1][2][3][4][5]. Nodal and parenchymal involvement are more common and endobronchial involvement as a primary presentation is uncommon [1][2][3][4]6,7].…”
