Preoperative statin therapy is associated with improved outcomes and resource utilization in patients undergoing aortic aneurysm repair

Abstract: With respect to both clinical outcome measures and subsequent resource utilization, statin therapy is associated with a beneficial effect in patients undergoing elective AAA repair. These data suggest that preoperative statin therapy should be an integral part of the risk optimization for patients undergoing AAA repair.

“…Beyond their cholesterol lowering effect, statins stabilize plaque and prevent strokes through anti-inflammatory and other mechanisms [1]. Moreover, they have been shown to be useful in treating aortic aneurysms [2], inflammatory colitis [3], chronic renal disease [4], nasal polyp disease [5] and chronic lymphocytic leukemia [6]. It has also been proven that statins have an anticancer effect due to suppression of the mevalonate biosynthesis which is a precursor for isoprenoid intermediates, such as farnesyl pyrophosphate and geranylgeranyl pyrophosphate, the important lipid moieties added during post-translational modification of a variety of proteins such as G-protein subunits Ras and Rho.…”

“…2,3) In this endovascular era, we have sought a neoadjuvant therapy for EVAR, and recent reports have revealed that preoperative statins are beneficial in patients undergoing aortic aneurysm repair without reference to the relationship between the operative maneuvers and atheromatous shower. 18) It had been considered that the beneficial effects of statins are mainly due to cholesterol reduction. But some reports have revealed another mechanisms independent of LDL-C reduction.…”

“…115 Statins have also been associated with a decreased risk of complications after endovascular repair of AAA and a decreased risk of stroke after carotid stenting. 116,117 Observational series suggest that perioperative statin therapy is also associated with a lower risk of acute renal failure and with lower mortality in patients experiencing post-operative complications or multiple organ dysfunction syndrome. 114 Statins may decrease the risk of post-operative atrial fibrillation (AF) following major non-cardiac surgery.…”
