Objective:To explore the relationship of the perioperative B-type natriuretic peptide (BNP) level with heart function among patients undergoing on-pump coronary artery bypass graft surgery on a beating heart.Methods:Total 90 patients expected to undergo coronary artery bypass graft surgery were selected and their left ventricular ejection fraction (LVEF) were examined before operation. Patients with LVEF greater than or equal to 50% were selected as the A group (n=46), and those less than 50% formed the B group (n=44). BNP levels of the patients were examined and its relationship with cardiac function was analyzed.Results:BNP levels of group A was lower than that in group B pre-and post-operatively (until 7 days after the surgery), the difference is statistically significant (p<0.05). Pearson analysis showed that the BNP level was negatively correlated with the LVEF (r = 0.767, p< 0.05). The area under the Roc curve is 0.865.Conclusion:BNP level was negatively correlated with the LVEF. Perioperative BNP level can be used as the prediction for heart function of patients with on-pump coronary artery bypass graft surgery on a beating heart.

Introduction: Postoperative atrial fibrillation (POAF) is common after cardiac surgery and associated with increased hospital length of stay, patient morbidity and mortality. We aimed to identify factors associated with POAF and evaluate the accuracy of available POAF prediction models. Methods: We screened articles from Ovid MEDLINE® and PubMed Central® (PMC) and included studies that evaluated risk factors associated with POAF or studies that designed or validated POAF prediction models. We only included studies in cardiac surgical patients with sample size n ⩾ 50 and a POAF outcome group ⩾20. We summarised factors that were associated with POAF and assessed prediction model performance by reviewing reported calibration and discriminative ability. Results: We reviewed 232 studies. Of these, 142 fulfilled the inclusion criteria. Age was frequently found to be associated with POAF, while most other variables showed contradictory findings, or were assessed in few studies. Overall, 15 studies specifically developed and/or validated 12 prediction models. Of these, all showed poor discrimination or absent calibration in predicting POAF in externally validated cohorts. Conclusions: Except for age, reporting of factors associated with POAF is inconsistent and often contradictory. Prediction models have low discrimination, missing calibration statistics, are at risk of bias and show limited clinical applicability. This suggests the need for studies that prospectively collect AF relevant data in large cohorts and then proceed to validate findings in external data sets.

Giriş: Postoperatif atriyal fi brilasyon (AF) kardiyak operasyonlar sonrasında mortalite ve morbiditeye etki eden önemli bir faktördür. Bu çalışmada, kliniğimizde yapılan koroner baypas operasyonları sonrası AF geliş-me sıklığının ve risk faktörlerinin araştırılması amaçlanmıştır. Hastalar ve Yöntem:Kliniğimizde Ocak 2011-Nisan 2011 yılları arasında yapılan koroner baypas operasyonlarının kayıtları retrospektif olarak incelenmiştir. Çalışmaya 211 hasta dahil edilmiştir. Postoperatif AF gelişen hastalar Grup I, gelişmeyen hastalar ise Grup II olarak sınıfl andırılmıştır. İki grup, diabetes mellitus (DM), hipertansiyon (HT), sigara kullanımı, kronik obstrüktif akciğer hastalığı (KOAH), distal anastomoz sayısı ve aort kros klemp süresi (AKK) açısından karşılaştırılmıştır.Bulgular: Grup I'deki hastaların 33 (%68)'ü erkek, 15 (%32)'i kadın hasta, Grup II'deki hastaların 119 (%73)'u erkek, 44 (%27)'ü kadındı. Ortalama yaş Grup I'de 66.2 ± 10.8 yıl, Grup II'de 60.4 ± 9.3 yıl olarak saptandı (p< 0.05). Gruplar arasında DM, sigara alışkanlığı, HT ve KOAH açısından anlamlı fark yoktu. Distal anastomoz sayısı Grup I'de ortalama 5.6 ± 2.1, Grup II'de ise ortalama 3.5 ± 2.4 idi (p< 0.05). AKK süresi I. Grupta ortalama 58.2 ± 29.1 dakika, II. Grupta ortalama 39.6 ± 18.4 dakika olarak bulundu (p< 0.05). Ortalama yatış süresi Grup I'de 5.2 ± 2.6 gün, Grup II'de 4.5 ± 3.1 gün olarak belirlendi. Erken dönem mortalite izlenmedi (Taburculuk sonrası ilk bir ay). Yaş, AKK ve distal anastomoz sayısı, postoperatif AF gelişimini etkileyen temel faktörler olarak belirlendi. The two groups were compared in terms of a number of potential risk factors for AF including the presence of diabetes mellitus (DM), hypertension (HT), cigarette smoking, chronic obstructive pulmonary disease (COPD), number of distal anastomoses, and aortic cross clamp time (ACC).Results: There were 33 (68%) were male and 15 (32%) female patients in Group I and 119 (73%) male and 44 (27%) female patients in Group II, respectively. Mean age was 66.2 ± 10.8 years in Group I and 60.4 ± 9.3 years in Group II (p= 0.001). There were no signifi cant differences between study groups in terms of the incidence of DM, smoking, HT or COPD.
