Prenatal sonographic diagnosis of single umbilical artery

Abstract: Purpose We evaluated the reliability of prenatal sonography and the usefulness of the umbilical vein and artery diameter ratio, transverse umbilical artery diameter, and systolic‐diastolic ratio in detecting single umbilical artery (SUA). Methods In 24 of 6,970 fetuses, SUA was detected using prenatal sonography and confirmed at birth. These cases were retrospectively reviewed. Results Four fetuses had major malformations, and 1 had a minor anomaly. Thirteen fetuses with SUA underwent chromosomal analysis, and… Show more

“…Owing to the lack of population‐based studies, we summarized the results of case series on aneuploidy in iSUA fetuses (Table 1). Among 695 iSUA cases reported on in 16 studies, there were four cases of aneuploidy (0.58%). The average maternal age in these studies was 30.1 years.…”

“…Therefore there is no certainty about the true number of iSUAs. As the reported specificity of prenatal ultrasound in detecting SUA is high (average 98.9 (range, 93.4–100.0)%), it is unlikely that TVC fetuses in the SUA group positively biased the outcome findings.…”

“…The literature includes conflicting recommendations regarding management when SUA is recognized prenatally. 1,3,4,[11][12][13][14][15][16] There is general agreement that targeted ultrasound, if not already done, is indicated due to the high incidence of associated anomalies, 1,3,4,13,15,16 but many believe that if no other anomalies are detected on ultrasound, further testing is unnecessary.…”

“…However, in the presence of concurrent structural anomalies, chances of chromosomal anomalies being present are high. [48] The incidence of single umbilical artery is 0.2–1%. Probable etiologies include primary agenesis of one of the arteries, secondary atrophy of a previously normal artery, or persistence of the original single allantoic artery of the body stalk.…”
