A diferenciação anatômica, psíquica e emocional entre os gêneros masculino e feminino é bastante complexa e envolve os fenômenos biológicos referentes à genética e à ação dos esteroides sexuais, além dos fenômenos epigenéticos, os quais atuam simultaneamente para promover ou eliminar a diferenciação dos circuitos neurais cerebrais entre os sexos 1 . Em especial, o conhecimento sobre os aspectos biológicos da formação da identidade de gênero é bastante restrito, uma vez que a maioria dos estudos sobre a anatomia e a influência hormonal nesse processo é baseada em experimentos com animais extrapolados para humanos. Já a influência do componente psíquico, sob o controle do processo de socialização do indivíduo, supostamente, faz parte da definição da identidade de gênero; entretanto, isso ainda é um assunto controverso.Identidade de gênero significa a consciência que a pessoa tem de ser homem ou mulher 2 , e quando ocorrem alterações nessa diferenciação ocorre o transexualismo, definido como sendo a incongruência entre o fenótipo (masculino ou feminino) e o sentimento de ser um ser masculino ou feminino 2 .As diferenças sexuais resultam do desequilíbrio inerente aos genes codificados pelos cromossomas sexuais (X,Y). No braço curto do cromossoma Y está localizado o gene SRY, o qual determina a formação da gônada masculina 3 . Esse gene se expressa especificamente nos neurônios que expressam a enzima tirosina hidroxilase da substância negra cerebral. Em estudo experimental com ratos machos, a supressão do SRY resulta em déficit motor, embora o número de neurônios permaneça o mesmo 4 , o que sugere o envolvimento desse gene em importantes propriedades bioquímicas dos neurônios dopaminérgicos do sistema nigroestrial no sexo masculino. As células cerebrais da mulher e do homem apresentam diferenças nos padrões de expressão de outros genes que são específicos para o cérebro em desenvolvimento, os quais determinam funções e habilidades específicas para cada gênero 1 .Do ponto de vista anatômico, as diferenças entre os cérebros masculino e feminino estão relacionadas às dimensões de regiões específicas. Durante o desenvolvimento do concepto, os núcleos do dimorfismo sexual e periventricular anteroventral da área pré-optica inicialmente contêm o mesmo número de neurônios. Entretanto, com o aumento na apoptose celular mediada pelo estradiol na mulher, o núcleo do dimorfismo sexual torna-se menor.
