Prenatal diagnosis of single umbilical artery: determination of the absent side, associated anomalies, Doppler findings and perinatal outcome

Abstract: Our data suggest that in fetuses with single umbilical artery the absence of the left artery is more frequent than the absence of the right artery. The association with additional malformations seems to be equal on each side.

“…The literature includes conflicting recommendations regarding management when SUA is recognized prenatally. 1,3,4,[11][12][13][14][15][16] There is general agreement that targeted ultrasound, if not already done, is indicated due to the high incidence of associated anomalies, 1,3,4,13,15,16 but many believe that if no other anomalies are detected on ultrasound, further testing is unnecessary.…”

“…9 Similar anomalies have been noted on ultrasound among cases ascertained prenatally. 1,3 We decided to use pathology as the means of ascertainment because we felt it was more accurate than ultrasound, despite recent advances with color imaging. Also we wanted data applicable to all stages of pregnancy, including the second trimester when decisions about amniocentesis and pregnancy intervention are being made.…”

“…Eleven series are based on cases diagnosed prenatally and significant differences in birth weight were demonstrated in four out of six studies that included control groups. 6,7,11,[13][14][15]18,[20][21][22][23] On the other hand, all post-natal studies have shown that birth weight is significantly lower in ISUA pregnancies compared to control pregnancies. 5,9,10,12 This is possibly related to the fact that these are the studies with greater sample size.…”

“…[1][2][3][4] However, despite considerable work on the pathologic, embryologic, and clinical aspects of this vascular malformation, the etiology of SUA still remains unknown. The 3 suggested mechanisms by which SUA could develop include primary agenesis, secondary atrophy, and persistence of the transient single artery normally present in early stages of development.…”
