Prenatal diagnosis of fetal adrenal hemorrhage and endocrinologic evaluation

Abstract: We present a case of a fetal adrenal hemorrhage, a rare disease in fetal life, detected prenatally at 36 weeks' gestation by ultrasound. Routine ultrasound examination at 36 weeks' gestation by primary obstetrician showed a cyst on the fetal suprarenal area. Initially, the suspected diagnosis was a fetal adrenal hemorrhage, but we should diagnose differently from neuroblastoma. Subsequent ultrasound examination at 38 and 39 weeks' gestation showed increase of the cyst in size. A 3.34-kg-male neonate was born b… Show more

“…[ 3 ] The main differential diagnosis of suprarenal masses in the fetus and neonate is congenital neuroblastoma, lung sequestrations, adrenal hemorrhage, mesoblastic nephroma, and duplications of the urinary or intestinal tract. [ 1 ] However, neuroblastoma is the most important differential requiring immediate attention due to its notorious extension, need for urgent therapy, and prognostic implications.…”

“…Risk factors associated with adrenal hemorrhage in utero include maternal hypertension, maternal diabetes, Beckwith–Wiedemann syndrome, asphyxia, shock, infections, thrombosis of inferior vena cava and left renal vein, and hemorrhagic disorders. [ 1 2 ] Although neonatal hemorrhage is common, accounting for 1.7–2.1 per thousand births, the incidence of antenatal hemorrhage is not elicited in any study and is rarely picked up in antenatal sonography. [ 3 ] Sonographic appearance can range from simply bulky echogenic adrenals to completely cystic lesion.…”

“…Elevated levels of amniotic fluid catecholamines support the diagnosis since these tumors usually produce and secrete catecholamines. (4) Adrenal hemorrhage [ 53 , 54 , 55 , 56 ]: the mass has heterogeneous echoes with a poorly-defined border. Color flow mapping shows no flow in the mass.…”

“…E tünetek és laboreltérések azonban csak mindkét mellékvese állományának több mint 90%-os pusztulása következtében alakulnak ki. Kisebb mértékű nekrózis esetén csökkent kortizol-és emelkedett ACTH-, valamint reninaktivitás ellenére klinikai elváltozások nem mutatkoznak [23]. A mellékvese regenerációs képessége jó.…”
