Objetivo: compreender as vivências de mães de crianças com Síndrome de Down após o diagnóstico de Cardiopatia Congênita infantil. Método: estudo exploratório e descritivo, qualitativo, realizado por meio de entrevistas semiestruturadas com nove mães de crianças com Síndrome de Down e Cardiopatia Congênita, com dados tratados pela análise temática de conteúdo de Bardin, à luz do referencial teórico da Teoria das Representações Sociais. Resultados: a categoria principal "Vivências maternas após o diagnóstico de cardiopatia congênita no filho com Síndrome de Down" compôs-se de quatro subcategorias, que retrataram os desafios enfrentados pelas mães, desde a comunicação do diagnóstico, às reações emocionais, à maternagem e ao tratamento da condição. Considerações finais: a vivência materna com um filho com ambos os diagnósticos mostrou-se desafiadora, com uma experiência acumulativa de sofrimento.

Congenital heart disease (CHD) is a structural abnormality in the heart or major intrathoracic blood vessels that has been present since birth. The antenatal detection of congenital cardiac disease has been greatly enhanced by the advent of fetal echocardiography as a crucial component of prenatal ultrasound evaluation. Nevertheless, antenatal CHD diagnosis rates are still lower than those for the majority of other significant structural defects. Aim : Assess the effectiveness of fetal in comparison to neonatal echocardiography. This study is conductedin accordance to the PRISMA statement. Studies were identified from several open-access electronic databases (PubMed Central, ScienceDirect, Google Scholar). Risk of bias of each study was evaluated using Cochrane Risk of Bias In Non-randomized Studies of Interventions (ROBINS-I) tool. Data were descriptively examined and narratively reported. Twelve studies were included in the review. All included studies were considered low-risk. From 12 studies that were included in our study, all recommend fetal echocardiography for prenatal assessment. Lowest reported sensitivity was 64.5%, highest value was 100%. Lowest repoted specificity was 88.9%, highest value was 99.96%. Diagnostic accuracy was reported in 4 studies, with a value of 93 – 99.82% Factors that might be associated with the accuracy of fetal echocardiography are high anatomic complexity, maternal comorbidities, and fellow as initial imager. Fetal echocardiography was found to have a high specificity but limited sensitivity. Low sensitivity suggests that fetal echocardiography results could be inaccurate whereas high specificity means that a negative echocardiography result is often sufficient to predict the absence of CHD. There are some factors that may affect the accuracy of fetal echocardiography, mostly resulting from fetal or maternal factors, such as high complexity of the anomaly, fetal position, late gestation, maternal obesity, and less-esperienced sonographer.

Introduction: CHDs are the most common type of birth defect. One in four newborns with a heart defect has a critical CHD. In Mexico, there is a lack of data available to determine its prevalence. Pulse oximetry screening programmes have been implemented worldwide, reporting opportunity areas in algorithm interpretation and data management. Our study aims to share preliminary results of a 3-year experience of a multicentre pulse oximetry screening programme that addresses critical challenges. Materials and methods: This retrospective study examined the reports of newborns screened from February 2016 to July 2019 from five hospitals. Two algorithms –the New Jersey and the American Academy of Pediatrics– were implemented over consecutive periods. The algorithms’ impact was assessed through the calculation of the false-positive rate in an eligible population. Results: A total of 8960 newborns were eligible for the study; from it, 32.27% were screened under the New Jersey and 67.72% under the American Academy of Pediatrics algorithm – false-positive rate: 1% (CI 95: ± 0.36%) and 0.71% (CI 95: ± 0.21%), respectively. Seventy-nine newborns were referred, six were diagnosed with critical CHD, and six with CHD. The critical CHD estimated prevalence was 6.69:10,000 newborns (CI 95: ± 5.36). Our results showed that the algorithm was not related to the observable false-positive rate reduction. Discussion: Other factors may play a role in decreasing the false-positive rate. Our experience implementing this programme was that a systematic screening process led to more confident results, newborn’s report interpretation, and follow-up.
