Preliminary results of video-assisted anal fistula treatment (VAAFT) in children

Abstract: VAAFT proved to be feasible and safe in children. It also proved to be versatile as it could be applied to fistulas of different etiologies. The key to success seems to be an adequate mucosal sleeve. Older children and adolescents benefit most from VAAFT which is a valid alternative to available surgical procedures.

“…The VAAFT technique can preserve the anal sphincter because the fistula is treated by endoscopy without fistulotomy or fistulectomy, efficiently avoiding injury to the anal sphincter. Recently, many scholars have begun to implement this method to treat anal fistulae, 9,13-16 including pediatric anal fistulae and anal fistulae associated with Crohn’s disease. The results exhibited in these studies support the conclusion that VAAFT is a safe and efficient surgical method to preserve the anal sphincter and an alternative approach for anal fistula treatment.…”

“…At a mean follow‐up of 9 months 9 of 11 patients, where VAAFT was technically possible, were deemed a success, and no deterioration in continence was observed. VAAFT has also been reported in a paediatric population . Thirteen procedures were performed on nine children (mean age 9.6 years).…”

“…The procedure rapidly gained popularity worldwide; it is also expanded [60] in the pediatric population and anal CD, which may be combined [61] with RAF. There is [62] a palliative effect in the main local anal symptoms, such as pain and discharge, in patients with anal CD and complex refractory AF undergoing VAAFT.…”
