Preliminary Findings from the National Register of Antipsychotic Medication in Pregnancy

Abstract: The establishment of NRAMP is an important strategy in improving the management of serious mental illness such as schizophrenia and related disorders, in women who are pregnant. This project involves extensive collaboration between many different clinical groups and industry, and shall culminate in an important resource to improve the quality of life for both patients and future generations.

“…It seems prudent to suggest that pregnant women treated with olanzapine should be offered an oral glucose tolerance test, particularly if they have a raised BMI: indeed, in light of recent research finding an increased risk of gestational diabetes in women prescribed antipsychotics in pregnancy [Boden et al 2012], we would suggest all women prescribed antipsychotics with raised prepregnancy/first trimester BMI should be offered this test. In addition, it would be potentially helpful to consider the creation of an international register to monitor routinely the maternal and foetal outcomes of antipsychotic use in pregnancy [Kulkarni et al 2008].…”

“…Gonadal steroid hormones act in the CNS to affect the drug response and are different in men and women (Kulkarni et al, 2008b). Tardive dyskinesia is seen more commonly in women than men (Usall et al, 2007;Yassa and Jeste, 1992).…”

“…An evidence-based guideline for antipsychotic prescribing during pregnancy is needed. The Australian National Register of Antipsychotic Medication in Pregnancy (NRAMP) will provide local data towards developing such guidance (Kulkarni et al, 2008b(Kulkarni et al, , 2014b). An NRAMP prospective cohort study (Kulkarni et al, 2014b) found that cessation of antipsychotic medication during pregnancy was associated with relapse of psychosis, maternal hospitalisation and the subsequent separation of mother and baby, leading to attachment and bonding problems.…”
