Preliminary experience with the Pipeline Flex Embolization Device: technical note

Abstract: The Pipeline Flex device allows more precise and controlled deployment than the current PED device. Although this preliminary experience seems positive, multicenter larger series will be needed to confirm the safety and durability of this new device.

“…However, the delivery system has been redesigned with several important modifications. These have been previously described in detail 7 8. Briefly, the distal end of the device is no longer constrained on a capture coil and does not require torque to release.…”

“…Briefly, the distal end of the device is no longer constrained on a capture coil and does not require torque to release. Rather, there are two polytetrafluoroethylene flaps that protect the distal braid and facilitate opening when purposely inverted 7. The proximal device is also no longer constrained, and it is now positioned on a resheathing pad.…”

“…Since then, only two reports8 9 have described it, with particular emphasis on the technical aspects. Although the stent has not changed, the new delivery system makes the device resheathable, theoretically allowing a more precise, safe and efficient deployment.…”

“…In our patient, the resistance was so strong that we could not even advance the delivery wire let alone attempt the maneuver to release the protective sleeves. Martínez-Galdámez et al 10) recommended to recover the entire system if more resistance is felt in the manipulation of the delivery wire than usual. However, the PED is an expensive device, and repeated trials should be avoided also from the viewpoint of medical economy.…”
