Introduction:The incidence of grand multiparty in Saudi Arabia is high; there are no adequate studies about the grand multiparty.Aim:The aim to determine the prevalence, fetal and maternal outcomes in grand multipara women in comparison to primipara’s women.Methods:A retrospective study, the data was collected from our hospital records of labour and delivery unit started from January 2016 to December 2016.Results:The analysis was done in 415 charts divided into two group 1 consist of (120 primigravidas) used as the control and group 2 the study group (295 grand multiparas). When comparing the maternal age between the two groups, grand multipara was older (mean of 36.9 ± 4), and primigravida age (33.5 ± 5.7) with a p-value < 0.001. The fetal weight, Gestational age, and Apgar score at 1, and 5 min, the type of delivery, medical disease are common in grand multipara and statistically significant P<0.001. However, HTN is more common in primigravida but not statistically significant with P value <0.287. Premature rupture of membrane. Intrauterine growth retardation breech presentation and preterm labour, Placental abnormality and postpartum haemorrhage more common in grand multipara and statistically significant a p-value < 0.001. The neonatal outcome, ICU admission, intrauterine fetal death all were similar in each group, not statistically significant. Logistic regression performed of a different variable.Conclusion:Grand multipara is common in our institution and the outcome of medical diseases, and obstetrical complication is more in grand multipara than primigravida.

BackgroundDespite the high prevalence of premature rupture of membranes (PROM) in low-resource settings, the preferred mode of delivery remains unclear. We compared the perinatal mortality in a prospective cohort of women with PROM after 28 weeks following vaginal or caesarean delivery at Mulago Hospital with the aim of adopting evidence based practice and improving patient care.MethodsBetween November 2015 and May 2016, 1455 women with PROM after 28 weeks of gestation and their newborns were prospectively followed from admission to discharge at Mulago Hospital. The primary outcome was perinatal mortality. Secondary neonatal outcomes included sepsis and admission to the Special Care Unit. Maternal outcomes included maternal deaths and complications. Outcomes were compared between women who had vaginal vs. caesarean delivery using multivariable logistic regression. All statistical tests were 2-sided with the level of statistical significance set at p < 0.05.ResultsThe incidence of PROM was 12.1%. The perinatal mortality following PROM was 65 per 1000 live births. Of the 1425 women with PROM, 991 (69.5%) had vaginal delivery and 434 (30.5%) underwent Caesarean section. There was no statistical difference in perinatal mortality by the mode of delivery (vaginal vs. caesarean) in PROM (p = 0.12). The risk factors for perinatal mortality included chorioamnionitis, failure to administer corticosteroids in preterm PROM, gestational age (28–33 weeks), duration of drainage of liquor (24–48 hours), and presence of maternal complications. Caesarean delivery was associated with increased maternal postpartum infections, admission to the Special Care Unit and maternal death.ConclusionIn low resource settings, vaginal delivery is the preferred mode of delivery for PROM after 28 weeks gestation. It is associated with lesser maternal and perinatal morbidity when compared to caesarean delivery.

The effect of antenatal magnesium sulfate on intraventricular hemorrhage in premature infants: a systematic review and meta-analysis

Objective The aim of this systematic review and meta-analysis study was to determine the pooled estimate of the effect of antenatal magnesium sulfate (MgSO 4 ) on intraventricular hemorrhage (IVH) in premature infants. Methods Two review authors independently searched all randomized clinical trials from international databases, including Medline (PubMed), Web of Sciences, Scopus, Cochrane Central Register of Controlled Trials (CENTRAL), and Research Registers of ongoing trials (ClinicalTrials.gov), from January 1989 to August 2017. Two independent review authors were responsible for data collection. After extracting the necessary information from the evaluated articles, metaanalysis of the data was performed using Stata version 14. Also, sources of heterogeneity among studies were determined by Meta regression. Results In this study, among 126 articles that were extracted from primary studies, 7 papers that evaluated the effect of MgSO 4 on IVH were eligible for inclusion in the meta-analysis. The results of the meta-analysis showed that pooled relative risk (95% confidence interval [CI]) was 0.80 (95% CI, 0.63 to 1.03) for the effect of MgSO 4 on IVH. Conclusion Results of this study showed that although MgSO 4 had a protective effect on IVH in premature infants, this effect was not statistically significant. Further studies are needed to determine the best dosage, timing, and gestational age to achieve the optimum effect of MgSO 4 on IVH.
