Pregnancy in a Non-Communicating Rudimentary Horn of Uterus: A Clinical Case Report

Abstract: Pregnancy in non-communicating rudimentary horn is extremely rare. It carries grave risk to mother and fetus due to rupture of rudimentary horn in second trimester of pregnancy. Here is a case of ruptured rudimentary horn pregnancy at gestation of 14 weeks. Laparotomy was done and the rudimentary horn was excised. Postoperative recovery was uneventful.   Keywords: Two unicornuate uteruses; rudimentary horn; rupture DOI: 10.3329/bmj.v39i1.6234 Bangladesh Medical Journal 2010; 39(1): 47-48

“…8 Ninety percent of rudimentary horns are noncommunicating to the main uterine cavity, and pregnancy in the rudimentary horn results from transperitoneal migration of the sperm or conceptus. 1,9 The majority of patients with Mü llerian anomalies are asymptomatic. Dysmenorrhea (80%), urinary-tract anomalies (60%), hematometra, and endometriosis are commonly associated with the presence of a rudimentary uterine horn because of the existence of functioning endometrium.…”

“…An early bimanual palpation showing a deviated uterus with a palpable adnexal mass extending outside the uterine angle (Baart de la faille's sign) should lead to a suspicion of this anomaly 6 . The availability of advanced ultrasound and magnetic resonance imaging makes the diagnosis of this condition possible at an early gestational age.…”

“…6 As the uterine wall is thicker and more vascular, bleeding is more severe in rudimentary horn pregnancy rupture. 7 Kadan and Romano described rudimentary horn rupture as the most significant threat to pregnancy and a lifethreatening situation. 8 Early diagnosis of the condition is essential and can be challenging.…”
