Ninety-one human oocytes, lacking signs of fertilization 50 h after insemination in vitro, were investigated cytogenetically to assess the frequency and type of chromosomal abnormalities. Chromosome spreading permitted adequate karyotyping in 55 oocytes. Non-determined numerical aberrations occurred with the following frequencies: hypohaploidy, 10.9% (6/55), hyperhapJoidy, 14.5% (8/55) and hyperdiploidy, 3.6% (2/55). Total aneuploidy occurred with a frequency of 29.1% and was observed in oocytes from 30 patients. No correlation was found between specific chromosomal aberrations and type of infertility, stimulation treatment or gonadotrophin levels. On the other hand, the frequency of aneuploidy was significantly higher (P < 0.05) in patients > 35 years of age. Two chromosomal complements (3.6%) had structural rearrangements; one oocyte had both structural and numerical chromosomal abnormalities and the other had differently condensed regions on the long arms of three chromosomes from group C. The overall frequency of chromosomal aberrations was 32.7%. Only two samples contained an additional set of polar body chromosomes. Thirteen oocytes presented sperm chromosomes in an arrested stage of premature chromosome condensation of the G, phase and four oocytes showed asynchronous condensation of pronuclear chromosomes. Finally, it was concluded that the high proportion of chromosomal aberrations observed in human oocytes may contribute significantly to abnormal embryonic development in vitro.

Thirty-seven patients underwent ultrasonographic, clinical, and hormonal monitoring of superovulated cycles in order to time HCG administration in an attempt to schedule the laparoscopy at the optimal time for oocyte recovery. Superovulation was induced by Clomiphene in patients under the age of 35, with both ovaries; Clomiphene and HMG were administered in women over age 35 and/or with only one or half an ovary. Twelve patients were excluded because of poor oestradiol values or LH rise prior to HCG administration. Four patients were excluded at laparoscopy because of inadequate ovarian access (a preliminary laparoscopy was not performed). In 21 patients follicular aspiration was performed: 33 mature, 15 immature, and 5 degenerated oocytes were obtained (1.5 mature oocyte per patient). Thirty eggs were fertilized on a total number of 48 eggs (mature and immature)-fertilization rate of 63%. Twenty-nine embryos cleaved and were transferred to 13 patients at the 4-to &cell stage.

Infertility is a serious hardship. There have been several advancements in its management. A major breakthrough in 1978 was the successful achievement of pregnancy and the birth of a girl as a result of In Vitro Fertilization (IVF). That success led to several technological innovations that heralded a new era of "Non Coital Reproduction". These methods are collectively known as "Assisted Reproductive Technologies (ARTs)." These are now available to women all across the USA and in many other countries. Thousands of couples are achieving pregnancies through the use of ARTs every year. Most of these couples would otherwise never have achieved pregnancies.Although this is a welcome addition to the medical armentarium, there are several unresolved ethical, societal and religious concerns about their applications, especially in regards to the more recent applications of IVF.A brief account of "normal" procreation and the basic principles of ARTs are given. Then the sharfa (Islamic law) principles that guide us in deciding the acceptability of these technologies will be discussed. Then the rulings that have been agreed upon by Islamic scholars will be given. Briefly stated, Islam not only allows but also encourages married couples to seek treatment of infertility, including the use of ARTs. However, certain limits cannot be crossed. The Islamic principles that identify these limits are 1) a valid marriage contract; 2) preservation of lineage including adopted offspring; 3) surrogacy is prohibited and the "birth mother" is the mother; and 4) gametes are not to be donated.
