Pregnancy-associated plasma protein A: spotlight on kidney diseases

Abstract: Pregnancy-associated plasma protein A (PAPP-A) is a biomarker routinely used in screening for Down syndrome in the fi rst trimester of pregnancy. It is also present in very small amounts in men and non-pregnant women. PAPP-A is a key regulator of local insulin-like growth factor (IGF) bioavailability -IGFs are essential for normal body size during fetal development, but they are associated with aging and agerelated diseases. Measurement of circulating PAPP-A can provide valuable information not only in pregnan… Show more

“…Interestingly, FR, which reduced cystic disease in the murine model, also significantly reduced Pappa mRNA expression. PAPP-A has been shown to correlate with renal function, be present in higher levels in patients on dialysis, and serve as an independent predictor of mortality of patients on hemodialysis (49,50,51). PAPP-A is also a marker of inflammation in dialysis patients (85,86).…”

“…PAPP-A is also involved in pathogenesis of several disease and is a therapeutic target in diseases such as atherosclerosis and cancer, as well as age-related diseases (43)(44)(45)(46)(47)(48). Plasma PAPP-A has also been shown to correlate with renal function, been present at higher levels in patients on dialysis, and serve as an independent predictor of mortality of patients on hemodialysis (49)(50)(51). PAPP-A is ubiquitously expressed in several organs in humans (41,(52)(53)(54)(55)(56) and highly expressed in the human placenta (57).…”

“…During pregnancy, high concentrations of PAPP-A are produced by trophoblasts and PAPP-A has been used in prenatal screening for genetic anomalies such as Down syndrome (7). Despite its name, PAPP-A is not specific to pregnancy; it is found in both males and non-pregnant females (8). Additionally, elevated PAPP-A levels have also been reported in patients with renal impairment, asthma and lung cancer (9)(10)(11).…”

“…It is a predictor of cardiovascular events among patients with cardiac chest pain and is associated with patients´ prognosis [10, 11]. Several studies documented increased PAPP-A levels in patients with chronic kidney disease [12]. Data from different patient cohorts with end-stage renal disease patients treated with long-term hemodialysis demonstrate the association of serum PAPP-A levels with adverse outcome [13-17].…”
