Purpose : To present a case involving the transfer of a single pronucleated oocyte resulting in a monozygotic twin pregnancy. Method : A descriptive case report of a single patient. Results : The patient conceived and was found to have a monochorionic diamnionic pregnancy which resulted in the birth of normal identical twin boys at 32 weeks of gestation. Conclusions : The case report addresses an issue that has not received proper attention in the literature. It illustrates that observing a single PN in an oocyte at fertilization check should not be an absolute deterrent to transferring the resulting embryo even in an older patient with a high FSH level. This report also suggests that single observations, especially at the assessment of fertilization, in the IVF laboratory are limited when evaluating embryo potential and normalcy.

Abstract. Studies of mammalian fertilization progressed very slowly in the beginning because of difficulties in obtaining a large quantity of fully mature eggs at one time. With progression of techniques to collect and handle eggs and spermatozoa, research in mammalian fertilization advanced rapidly. Today, far more papers are published on mammalian gametes and fertilization than those of all other animals combined. The development of assisted fertilization and related technologies revolutionized basic research as well as human reproductive medicine and animal husbandry. Reproduction is fundamental to human and animal lives. The author lists a few subjects of his personal interest for further development of basic and applied research of gametes and fertilization. Each reader will probably have more exciting subjects of future investigation. Key words: Assisted fertilization, Egg, Fertilization, ICSI, Sperm Discovery of Mammalian Eggs, Spermatozoa and Fertilization Although it was known from ancient times that the eggs of birds and frogs were the start of new life, no one was certain whether this was true for mammals until Karl von Bare 1792-1876 (1827) discovered tiny eggs within dog and human ovaries. Interestingly, spermatozoa had been discovered 150 years earlier by Anton van Leeuwenhoek 1632-1723 (~1676)* and a medical student of Leiden, Johan Ham who saw many tiny tadpole-like creatures ("animacules") within human seminal plasma. Leewenhoek later saw same creatures in the semen of the rabbit, dog and fish. He thought that they were normal components of semen originating from the testis. Obviously, Leeuwenhoek saw spermatozoa without knowing their exact biological role. It was not until 1840s-1870s when it became clear that spermatozoa and eggs of all animals, large or small, are cells [Albert von Koelliker 1817-1905 (1840) ; Karl Gegenbaur (1861)] and that sperm entry into the egg -in particular the union of sperm and egg nuclei (sygamy) -is the essence of fertilization and provides cytological evidence of biparental inheritance [Edouard van Beneden 1846-1910 (1875) Studies of Mammalian FertilizationThe analysis of animal fertilization began from the start of the 20 th century. Because invertebrate animals such as sea urchins and starfish produce millions of mature eggs at one time and their fertilization and embryo development can be followed readily in a dish of seawater, most of the early and important investigations of fertilization were done using gametes of aquatic invertebrate animals. In mammals, even after induction of superovulation, only a few eggs (usually up to 40-50) can be obtained from a single female. Furthermore, eggs are normally fertilized and develop within the female's body, which makes observations and experiments of eggs cumbersome. Microinsemination is a revolutionary and promising technique for the treatment of severe male infertility, such as severe oligozoospermia, asthenozoospermia, and the case of unexplained unfertilization in which the conventional IVF is ineffective even if semen parameters are normal. Several different microinsemination techniques, such as zona opening [1][2][3], subzona sperm injection [4, 5], and intracytoplasmic sperm injection (ICSI) [6], have been introduced. ICSI is considered the most effective technique, because ICSI has the highest fertilization rate and can be performed with the smallest number of spermatozoa. In addition, this technique is effective for functional disorders, such as disorders of capacitation, acrosome reaction, and spermegg fusion. ICSI was first reported in 1992 [6], and for the above reasons became a powerful fertilization tool in only three or four years [7, 8].At first, ICSI was indicated for the treatment of severe infertility in which the probability of pregnancy was nonexistent or very law. Its indications were then expanded to include azoospermia, and it has been fairly effective [9][10][11], but in such cases chromosomal and genetic abnormalities may be passed on to children [12][13][14]. In addition, it has been clarified that ICSI is not effective in some cases, such as necrozoospermia [15], in which there are no viable spermatozoa; immotile spermatozoa [16], in which viable spermatozoa cannot always be picked up; cases in which spermatozoa have the disorder of oocyte activation factor; and azoospermia, in which mature spermatozoa cannot be found in the testes. The present paper concerns issues that must be considered in the performance of ICSI, as well as limitations to the effectiveness of ICSI. -ReviewNecrozoospermiaIn necrozoospermia, every spermatozoa on in semen is found to be dead when analyzed by vital staining (eosin stain [17], FertiLight stain (Molecular Probes, Eugene, OR) [18]). Sperm-injected oocytes are less likely to develop when injected with dead spermatozoa than with motile spermatozoa. The rate of fertilization for ICSI ranges from 0 to 11% [15]. Since 1992, 659 patients have been treated for infertility, a total of 1,317 treatment cycles at our hospital, and there had been six treatment cycles for necrozoospermia (3 patients). Among these cases, the rate of fertilization was 33.3% (5/15 oocytes), but there were no pregnancies. Since the fertilization rate was very low, dead (nonviable) spermatozoa may reduce the oocyte activating factor or have fragmented nuclear DNA or have chromosome disintegration [19]. Even when the sperm factor is injured and nuclear DNA is damaged, fertilization is possible with ICSI, but we cannot get well developed embryos from these spermatozoa, so that there is no hope of pregnancy. In humans, the development of fertilized oocytes to 4-cell stage embryos is controlled by the maternal genes rather than the paternal ones [20], so that fertilized oocytes can develop to this stage regardless of the state of the paternal DNA.
