Preexisting Antibodies to an F(ab′)2Antibody Therapeutic and Novel Method for Immunogenicity Assessment

Abstract: Anti-therapeutic antibodies (ATAs) may impact drug exposure and activity and induce immune complex mediated toxicity; therefore the accurate measurement of ATA is important for the analysis of drug safety and efficacy. Preexisting ATAs to the hinge region of anti-Delta like ligand 4 (anti-DLL4) F(ab′)2, a potential antitumor therapeutic, were detected in cynomolgus monkey serum, which presented a challenge in developing assays for detecting treatment induced ATA. A total ATA assay was developed using a bridgin… Show more

“…Anti-hinge autoantibodies can be found in healthy individuals and patients with inflammatory diseases [ 5 , 27 ]. Indeed, purified autoantibodies prepared from serum IgGs using immobilized F(ab’) 2 generated from IdeS-cleaved IgG-P as the absorbent or using immobilized peptide possessing the “…PAPELLG” sequence with the free C-terminal glycine showed modest restoration of ADCC activity to scIgG-P in vitro.…”

“…toxins, viral proteins, and cytokines (13)(14)(15)(16)(17)(18). Naturally occurring antibodies to neoepitopes that are revealed after conformational change of a biotherapeutic with reactivity to the hinge region and/or other internal residues of the IgG have also been reported (19)(20)(21).…”

“…This unwelcome outcome indicated that AHAs to pepsin-generated F(ab′) 2 could present barriers for human therapeutic applications. 5 In contrast, platelet clearance was minimal when the papain-generated Fab fragment of mAb c7E3 was tested in humans (Figure 1(a)). 12 The immunological differentiation between Fab- and F(ab′) 2 -mediated platelet clearance revealed an unexpected type of cell eradication with potential relevance for cancer therapy.

“…A similar papain vs. pepsin cleavage site disparity was confirmed in a recent, independent study. 5 However, all of these findings still left open the question: why is AHA reactivity generated to IgG hinge cleavage sites arising from non-physiological proteinases? Attention turned to human and disease-related proteinases that might also cleave IgG in vivo .…”
