Preemptive analgesia in third molar impaction surgery

Abstract: Introduction:We have evaluated efficacy of diclofenac sodium as pre-emptive analgesia agent in a prospective triple blind placebo controlled randomized clinical trial in a patients undergoing third molar impaction surgery.Materials and Methods:Randomization of groups was done by randomization software and two groups were constituted one group receiving placebo pre operatively and then the drug for next five days while the other group was given diclofenac sodium pre operatively and then for five days. Results:Re… Show more

“…Preemptive analgesia may decrease post-operative pain in the clinic. In previous clinical studies, preemptive administration of an analgesic agent has been shown to improve postoperative pain control in case of laparoscopic radical prostatectomy [ 28 ], thoracotomy [ 29 ], and third molar extraction [ 30 31 ]. Additionally, preemptive application of 1% lidocaine for 10 minutes prior to L4 and L5 bilateral dorsal rhizotomy significantly attenuates hindlimb mechanical allodynia in patients with failed back surgery syndrome [ 32 ].…”

“…In the study by Bridgman et al diclofenac sodium did not demonstrate any significant pre-emptive analgesic effects ( 14 ). On the contrary Shah et al compared the analgesic effect of diclofenac sodium before and after third impacted molar surgery and suggested that preemptive medication provides better pain control ( 15 ). The findings in our study also support the notion to administer drugs preoperatively.…”

“…However, in certain pathologic skeletal disorders, such as inflammatory bone loss, both processes do not occur consecutively [ 15 ]. Osteocytes also play a key role in the formation and remodeling of bone [ 16 ]. They are mechano-transducer cells and crucial determinants of bone quality [ 16 ].…”
