Predisposing factors and incidence of newly diagnosed atrial fibrillation in an urban African community: insights from the Heart of Soweto Study

Abstract: These unique data suggest that urban Africans in Soweto develop AF at a relatively young age. Conventional strategies used to manage and treat AF need to be carefully evaluated in this setting.

“…Although the prevalence of diagnosed type 2 diabetes was relatively low (3.4%), 7% of patients (mainly obese women) were at risk of metabolic disorders on the basis of their blood glucose profile. Significantly, obesity is associated with other rarely reported forms of CVD in Soweto, with obese women in the Heart of Soweto cohort being more likely to present with atrial fibrillation [16]. Therefore, the potential impact of enhanced primary prevention in this context is obvious, particularly in truncating a future rise in hypertensive heart disease [17,18] as life-expectancy and unhealthy lifestyles escalate.…”

“…In South Africa (SA), the prevalence of AF in the urban black population has recently been documented to be 7% in a cardiovascular disease cohort (8% of heart failure patients, 4% of hypertensive patients, and 13% of valvular disease patients). [13] However, very little has been published on AF management outside the deve loped world. This prompted the Assessment of the Therapeutic Management of Patients with Atrial Fibrillation in South Africa (SAFIR-RSA).…”

“…Several studies have shown that this rhythm disorder is associated with higher risk of readmission in individuals with HF regardless of ejection fraction or rhythm control. 29,30 Recent findings from Africa suggest that AF occurs in 4.6% of cardiac cases (estimated 5.6 out of 100,000 cases/y) 31 The mean age of occurrence is 57.2 6 18.8 years in Africa compared with 70.1 6 13.4 years in North America. 32 Common risk factors include hypertension, HF, and valvular heart disease.…”
