Predictors of treatment requirement in HBeAg-negative chronic hepatitis B patients with persistently normal alanine aminotransferase and high serum HBV DNA levels

Abstract: Significant liver damage was detected in 40% of CHB patients with PNALT and high HBV DNA upon biopsy. Age and HBV DNA viral load were independent predictors of significant liver damage. A biopsy to determine the degree of liver damage is advisable for CHB patients older than 46 years.

“…When the studies carried out in Shanghai and Zhejiang are directly compared, the population of PNALT patients with an HAI score ranging from 4 to 6 is increasing. The situation in China aligns with observations in other Asian countries [2426], in which 35% of PNALT patients with mild and moderate necroinflammation were observed.…”

“…Treatment requirement was observed in 40% of the patients in total according to histology. These two differences in comparison with our study may be related with the fact that only patients with normal ALT levels were included in the study of Ormeci …”

“…In the study conducted by Örmeci et al from our country, HBeAg negative patients who underwent liver biopsy with a HBV DNA value of 2000 IU/mL and persistently normal ALT levels were included. The same criterion was used for significant liver injury (HAI ≥6 and/or stage ≥2 by Ishak's classification) as in our study.…”

“… 48 Moreover, in patients with normal serum ALT and levels of HBV DNA between 2000 and 20 000 IU/mL (so-called grey-zone patients), significant levels of fibrosis have been reported. 49 We have previously shown that patients considered ‘immune tolerant’ (HBeAg+chronic infection) had levels of fibrosis similar to those found in patients labelled immune active (HBeAg±chronic hepatitis), notwithstanding differences in biochemical and virological parameters. 50 …”
