Background: Evidence suggests that caregivers of children with sickle cell disease (SCD) normally oscillate between the use of herbal and conventional medicines but little evidence exists on the prevalence of and factors associated with use of both therapies in the extant literature. We examined factors associated with caregivers’ use of combined herbal and conventional medicine to treat children with SCD. Methods: This was a cross-sectional study conducted at Jinja Regional Referral Hospital in Eastern Uganda between January and March 2022. Caregivers of children with SCD aged 1 to <18 years attending the Sickle Cell Clinic at the hospital were interviewed using structured questionnaires. We collected data on caregivers’ socio-demographic characteristics, use of herbal or conventional medicine, perceptions of and intentions to use either or both therapies, self-reported use of either or both therapies, and community and health-related factors. A multivariable logistic regression model was computed to assess the factors independently associated with caregivers’ use of combined therapy to treat children with SCD, using Stata version 15.0. Results: Of the 372 caregivers interviewed, 341 (91.7%) were female. On average, Respondents aged 34.3 years (Standard Deviation [SD]: ±9.8 years). Thirty-seven percent (n=138) of the caregivers reported the use of both herbal and conventional medicine, 58.3% (n=217) reported use of only conventional medicine, while 4.6% (n=17) reported use of herbal medicine only. Higher odds of using combination therapy were found in caregivers aged 60+ years (adjusted odds ratio [AOR] = 11.8; 95% CI: 1.2, 115.2), those with lower secondary education (AOR=6.2; 95% CI: 1.5, 26.0), those who believed in the safety and/or efficacy of herbal medicine (AOR=3.3; 95% CI: 1.5, 7.6) and those who thought that use of both therapies was safe (AOR=7.7; 95% CI: 3.5, 17.0). Conclusion: Slightly more than one-third of the caregivers reported use of combined herbal and conventional medicines to treat children with SCD, the majority of whom were of older age (>60%) and had lower secondary education. These findings suggest a need for targeted health promotion to educate caregivers about the dangers of using both herbal and conventional medicines in treating children with SCD in this setting. BackgroundDuring the COVID-19 pandemic, medical e-commerce (MEC) has provided a way for patients with chronic diseases to purchase drugs online to maintain social distancing, decrease the risk of infection and community transmission, and relieve the burden on medical resources. Therefore, research which seeks to elucidate the drivers of purchase intention (PI) among patients with chronic diseases in MEC is vital. This study extended the theory of planned behavior (TPB) by integrating the price value (PV) variable into the original TPB framework and explored the effect of PV on patients' PI in MEC during the coronavirus pandemic.MethodsEmpirical data was gathered from 414 Chinese participants. Structural equation modeling was applied to explore the mechanism of chronic patients' PI in MEC. In addition, this study also estimated the moderating effect of gender, income, and region and the mediating role of attitude (ATT), subjective norm (SN), and perceived behavioral control (PBC) between PV and PI.ResultsPatients' PI in MEC is significantly affected by ATT, SN, and PBC. PV delivers significant influence on ATT, SN, PBC, and PI, with PV having the strongest effect on ATT. Gender, income, and region can significantly moderate the relationship between PV and ATT.ConclusionThese findings can contribute to design targeted interventions to increase the adoption of MEC for patients with chronic diseases, decrease infection rates, and alleviate the strain on medical resources in the COVID-19 era.

Background Despite Traditional Chinese medicine’s (TCM) historical roles in Chinese society, few research has been investigated the nature of TCM practitioner–patient interactions. Improved communication skills among TCM practitioners will result higher-quality interactions and better clinical outcomes. Methods To investigate the changes in TCM practitioners’ communication practices after communications training focused on promoting their awareness of integrating a patient’s medical history from conventional medicine in TCM treatment, Eight registered Cantonese-speaking TCM practitioners in Hong Kong were randomly recruited from local clinics and randomised into control (n = 12) and experimental groups (n = 12), with a total of 24 consultations. The experimental group was given training focused on patient-centred communication, with an internationally recognised and communication framework validated in global consultation settings (i.e. the Calgary-Cambridge Guide) on how to take a patient’s medical history from conventional medicine and communicate diagnosis and treatment plans. Consultations before and after training were audio-recorded and rated. The efficacy of the training was evaluated by comparing the two groups before training (pre-test), immediately after training (post-test) and after a 3-month delay (delayed post-test). Using validated scales, the primary outcomes were measured for the practitioners’ clinical communication skills and the quality of interactions. Results The communication training significantly improved the TCM providers’ patient-centred communication and communication proficiency. The results indicate that the team developed an effective communication model for integrating TCM and conventional medicine in Hong Kong. The framework helps trained TCM practitioners to integrate their patients’ conventional medical history when delivering patient care. The findings shed light on how interpersonal relationships between TCM practitioners and patients can be constructed after communication training to better care for patients’ psychological concerns in addition to their physical needs. Conclusion Trained TCM practitioners can provide an integrated model that takes patients’ conventional medical history into account when delivering a holistic patient-centred care. The findings can enhance our understanding of better ways to train the future TCM practitioners and to develop a continuing professional training for the current TCM practitioners to expand our understanding of TCM communication in acute clinical contexts and, thus offer a firm evidence-based foundation upon which to develop communication strategies that improve their clinical cpractices.
