Predictors of prolonged air leak following pulmonary lobectomy

Abstract: Prolonged air leak is quite a common pulmonary complication after pulmonary lobectomy and can be treated conservatively. This complication significantly prolongs the length of hospitalization (P<0.01). We also conclude that COPD increases the risk of an air leak persisting for longer than 7 days.

“…A number of studies have assessed the risk factors of PAL. Male patients (5,17), patients with preoperative pulmonary dysfunction (such as emphysema and COPD) (8,18,19), pleural adhesion and less developed fissure (14,20) were prone to have PAL. The operation performed by different surgeons would also affect the incidence of PAL (4).…”

“…COPD is the common cause of spontaneous seconder pneumothorax and the most consistently identified risk factor for persistent air leak that is detected in 20% of COPD patients (1). Also it is reported in up to 45% of cases who have undergone lung volume reduction surgery due to pulmonary emphysema (5).…”

“…No universal consensus exist as to the exact duration of air leak which constitutes a prolonged air leak, but it is generally regarded to exist if it is present for more than 5 days(1-4) or 7 days(2, 5-7) after initial surgery. It is an important complication that results in increased length of stay (8)(9)(10)(11)(12)(13)(14)(15) and has been associated with other post-operative complications such as pneumonia (12,14,16), empyema (9,10,16) and ICU re-admission (12).…”
