“…The value of cardiac resynchronization therapy was debated, since the updated ESC HF guidelines state that cardiac resynchronization therapy with pacing/defibrillator should be considered in patients with sinus rhythm, with a QRS duration of ≥150 ms (irrespective of QRS morphology) and an EF of ≤35% to reduce the risk of HF hospitalization and the risk of premature death (level of evidence: Class IIa, level A) [6]. Potential surgical options were discussed, including preoperative predictors of recurrent secondary MR after undersized annuloplasty, including specifically mitral geometry [11]. According to the 2012 ESC/European Society of Cardio thoracic Surgeons guidelines for valve disease [12], indications for surgery remain controversial, mainly because of the persistence and high recurrence rate of MR after valve repair and the absence of evidence that surgery prolongs life.…”
