Background: The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic remains a constant challenge for healthcare systems and affects the well-being of many individuals. The objective of the current study is to determine the parameters that can predict post COVID-19 sequale and incidence of post Covid sequale among Covid survivor's patients attending post covid outpatient clinic. Patients and methods: This was a case control study carried out on 60 COVID-19 survivors whom were recovered from the acute illness and discharged for follow-up at the post covid out-patient clinic at Zagazig University. The sixty patients included in the study were divided to Group I: (Asymptomatic patients): include 30 patients whom were discharged for follow-up and in whom no significant symptoms or signs were reported during the study. Group II: (Symptomatic patients): include 30 patients whom were discharged for follow-up and in whom there were significant symptoms or signs during the study. Results: There is statistically significantly association between initial parameters high (CRP, D-dimer, ferritin, LDH, IL6) and low Lymphocytes. Post covid degree of lung affection in chest CT scan where Grade ≥5 (severe lung affection) prevailed in 63.3% and 20% in those with and without post covid syndrome, respectively, while Grade <2 (less sever lung affection) was more dominant in those without post covid (30% vs. 6.7%). Conclusion: Persistence of symptoms is common after the acute phase of COVID-19 infection. Post covid syndrome occurs regardless of the disease severity. High CRP, LDH, ferritn, D-dimer, and IL6, low Lymphocytes and degree of lung affection at diagnosis of acute covid19 may be good predictors for the occurrence of post covid syndrome.

Most Common Long COVID Physical Symptoms in Working Age Adults Who Experienced Mild COVID-19 Infection: A Scoping Review

Background: One-third of patients who recover from COVID-19 present with long COVID. Their symptoms are broad, affecting their physical functioning and, ultimately, their quality of life. Many of those individuals who develop long COVID, possibly from a mild COVID-19 infection, are in the 18–65 age group. This prolongation of malaise directly influences national workforce economies. Objectives: To summarise the commonly reported physical symptoms of long COVID in order to inform potential adjustments in healthcare for the employable population. Methods: The Embase, CINAHL, Medline, SCOPUS, and WHO COVID-19 databases were searched. The study selection process was based on the PRISMA guidelines. The extracted data were synthesised and presented narratively. Results: 7403 studies were accessed, comprising 60 cohort studies and 10 case series/studies, representing 289,213 patients who met our criteria. The most frequently reported physical symptoms were fatigue (92%), shortness of breath (SOB) (81.8%), muscle pain (43.6%), and joint pain (34.5%). Conclusions: The range of reported physical symptoms was broad and varied; the main ones being fatigue, breathlessness/SOB, and pain. Similarities observed between long COVID and other post-acute infection syndromes may help formulate protocols to manage and promote recovery for long COVID patients. Inconsistencies were evident, particularly with a lack of adherence to the standardised definitions of long COVID.

Description of Post-discharge Outcomes of Patients with COVID-19 in a Tertiary Referral Center in the Philippines

Background and Objectives The immediate post-discharge period of COVID-19 patients is a vulnerable state due to several complications that may arise during this time. Some patients get readmitted shortly after being discharged while others report persistence of symptoms, develop specialized needs, or observe a decline from their baseline functional capacity. Information on the outcomes of post-COVID discharge patients in our institution is currently lacking. This study described the outcomes of patients with COVID-19 after their discharge from the service areas of Philippine General Hospital. Methods This study is a retrospective chart review involving charts of all adult patients discharged from the PGH COVID service areas last August 2021 to October 2021. Data from their follow up consults at 1 week, 1 month, and 3 months post-discharge were reviewed. Baseline characteristics and post-discharge outcomes including post-COVID symptoms, special care needs, mortality, rehospitalization, emergency consult, level of dependence, and ability to return to work were assessed. Results A total of 171 patient charts were included. The mean age of patients was 53.7 years. Most were male (60.2%), unemployed (59.7%), non-smoker (55%), hypertensive (57.9%), diabetic (50.2%), and obese (50.2%). Most of them were oxygen requiring (80%) and with severe to critical COVID infection (72.5%) during admission. At 3 months post-discharge, 113 (66%) were stable and able to complete the follow up, 8 (4.6%) died, 9 (5.2%) got readmitted, and 41 (23.9%) were lost to follow up. Among those who were able to follow up after 3 months, 84 (74%) were asymptomatic. Among those who remained symptomatic, the most common symptoms were dyspnea, fatigue, and cough. After 3 months, 100 (88%) did not require special care needs, 100 (88%) were fully independent, and 45 (39.8%) were able to return to baseline work. Conclusions Despite the majority of patients having severe to critical COVID infection during admission, most were asymptomatic within 3 months post-discharge. In those who developed persistent symptoms, dyspnea, cough, and fatigue were the most common symptoms identified regardless of COVID severity. Majority did not require special care needs.
