Predictors of Left Ventricular Function Deterioration in Patients With Left Bundle Branch Block and Ejection Fraction >50%

Abstract: Resynchronization therapy has become standard of care in patients with left bundle branch block (LBBB), congestive heart failure (CHF), and low ejection fraction (EF). In order to characterize the left ventricular (LV) function evolution in patients with LBBB and baseline preserved LVEF, records of all patients who visited an academic echocardiography laboratory during a period of 4 years were retrospectively investigated. Patients were included if they had a baseline EF >50%, LBBB on surface electrocardiograp… Show more

“…In patients receiving pacemakers, there is a concern about the long‐term effects of non‐physiological right ventricular pacing. Although significant ventricular dysfunction in unselected patients develops rarely, 264 the main predictors of this unfavourable outcome include LVH and heart failure 265 . Two‐year data from the PACE trial suggest that biventricular pacing for bradycardia in patients with preserved ejection fraction may lead to more favourable outcomes as compared to right ventricular pacing alone 266 .…”

“…Thus, achieving the LVEF values ≤ 40% in patients with an initial EF of > 50% was considered as LVEF deterioration. LVEF was visually estimated in the apical 4- and 2-chamber views ( 11 ).…”

“…least the theoretical basis for surveillance monitoring for the development of LBBB-induced cardiomyopathy (Angheloiu et al, 2013;Vaillant et al, 2013). Once LBBB develops, women may need more vigilant evaluations at shorter QRS durations.…”
