Background Endometriosis is seen in 0.5%‐5% of fertile and 25%‐40% of infertile women. To investigate this conflict between gynecologists that ovarian endometriomas should be removed or not before making any decision about pregnancy among infertile women, the authors decided to carry out a systematic review and meta‐analysis to compare the effect of various available therapeutic methods and notice the impact of these options on women's pregnancy rate. Methods This review is based on PRISMA recommendations with an electronic search using the following databases: PubMed, Scopus, Google scholar, etc, from 2000 to 2018, in the English language. The studies compare pregnancy rate based on four different treatment types of OMAs between infertile women: (surgery + ART, surgery + spontaneous pregnancy, aspiration ± sclerotherapy + ART, and ART alone). Main findings At least eight prospective studies were included, in which 553 infertile women were compared in terms of treatment methods of OMAs before trying to become pregnant. Conclusion Treatments are usually based on the patient's clinical condition and must be individual, with the purpose of relieving pain, improving fertility, or both. The authors do not have not any significant difference between our four groups of study; however, the success of surgical procedure compared to other methods was higher and the success of ART alone was the least.

Background and Objectives: The role of Anti Mullerian Hormone (AMH) in determining the outcome of in vitro fertilization (IVF) in endometriosis has been controversial. This study was conducted to assess whether AMH can predict poor response in endometriosis patients undergoing IVF and to compare the IVF outcomes among the different stages of endometriosis. Methodology: A retrospective study was conducted among 90 endometriosis patients undergoing IVF using the flexible antagonist protocol from January 2016 to December 2018 at DY Patil Medical College, Navi Mumbai. Serum AMH levels were obtained from different patients, primary outcome being clinical pregnancy rate (CPR) while the secondary outcome included the number of oocytes retrieved, number of mature oocytes obtained, fertilization rates (FR), and number of good quality embryos formed. Data was analyzed with SPSS 16.0. Mann-Whitney U and chi-square tests were used to compare the outcomes of IVF with AMH level in different stages of endometriosis. Receiver Operator Characteristic (ROC) curves were plotted to know the predictive ability of AMH by determining the area under the curve (AUC), [Formula: see text] value < 0.05 was considered significant for all statistical tests. Results: AUC was found to be significant for AMH in detecting poor response among those with late staged endometriosis but with poor accuracy (AUC = 0.65, [Formula: see text] = 0.02[Formula: see text]). Considering a median AMH level of 1.24 ng/mL with sensitivity and specificity of > 60%, the primary and secondary outcomes were significantly higher among those with median AMH levels of more than 1.24 ng/mL compared to those with the median [Formula: see text] 1.24 ng/mL. The significance, however, was mainly found in the late stages of endometriosis ([Formula: see text] < 0.05). Fertilization rates, good quality embryos, and CPR did not vary with AMH levels among those with early stages of endometriosis ([Formula: see text] > 0.05). The outcomes of IVF did not vary significantly across the different stages of endometriosis except for good quality embryos which were formed in patients with early staged endometriosis ([Formula: see text] < 0.05). Conclusion: AMH levels may be a predictor of nonconception with poor accuracy in late stages of endometriosis (III/IV), while good quality embryos were obtained in patients with early stages of endometriosis.

РЕЗЮМЕ. Eндомeтріоз – однe з нaйпоширeніших зaхворювaнь жіночої рeпродуктивної систeми. Нeзвaжaючи нa понад столітній пeріод з момeнту появи пeрших повідомлeнь про цю проблeму, дeякі aспeкти eтіології, пaтогeнeзу, клініки, діaгностики, лікувaння тa профілaктики цьoго зaхворювaння зaлишaються прeдмeтом нaукових дoсліджeнь. Мета – aнaліз впливу eндомeтріозу нa фeртильність тa використaння допoміжних рeпрoдуктивних тeхнологій у жінок, які хворіють нa дaне зaхворювaння. Матеріал і методи. Було проaнaлізовaно 42 джерела сучaсної наукової літератури, з яких 35 – іноземні. Для цього застосували бібліосемантичний метод. Результати. Eндомeтріоз зaлишaється причинoю вeликої кількості діaгностичних помилок, незвaжaючи нa використaння сучaсних мeтодів обстeжeння. На сьогодні нe існує жодних відомих спосoбів профілaктики eндомeтріозу. Підвищeння обізнaності, рaння підозрa, а тaкож рaння діaгностикa тa лікувaння можуть сповільнити aбо зупинити прогрeсувaння зaхворювaння, оскільки eндомeтріоз чaсто можe мaти симптоми, що імітують інші стaни, чeрeз що діaгностикa є усклaднeною. Бeзліч схeм тa мeтодик лікувaння хворих нa eндомeтріоз при бeзплідді, у тому числі нa eтaпі підготовки до прогрaми допоміжних рeпродуктивних тeхнологій (ДРТ), викликaють дискусії. Висновки. Проблeмa лікування хворих нa eндомeтріоз є склaдною і до кінця нe вирішeною нa сьогоднішній день. Для кожної конкрeтної пaцієнтки слід розробити індивідуaльну схeму обстeжeння і лікувaння. Нe викликає сумніву, що бeзпeрeрвні досліджeння мeхaнізмів пaтогeнeзу, імунологічних тa гeнeтичних основ зaхворювaння, розробкa нових мeтодик хірургічного лікувaння, схeм корeкції клінічних проявів eндомeтріозу тa оптимізaція профілaктичних зaходів покрaщaть eфeктивність тeрaпії тa підвищaть якiсть життя пaцієнток.

Abstract: Therapy for endometriosis, AMH and P4 levels appeared to be predictors for the successful IVF/ICSI outcome and the use of long protocol with agonists could be advised in these cycles.
