Predictors of hypoglycemia in hospitalized patients with diabetes mellitus

Abstract: Hypoglycemia is common among hospitalized patients with diabetes mellitus (DM), and is associated with increased morbidity and mortality. Identify pre-admission risk factors associated with in-hospital hypoglycemia. Historical prospectively collected data of adult DM patients hospitalized to medical wards between 2011 and 2013. Hypoglycemia and serious hypoglycemia were defined as at least one blood glucose measurement ≤ 70 and < 54 mg/dl, respectively, during hospitalization. The primary outcome was in-hospit… Show more

“…A report of 792 patients from Israel revealed that the mean age of patients with hypoglycaemia was 73 ± 13 years and that 51% of men had hypoglycaemia. 6 These findings are similar to those in our study of 186 patients in that the mean age was 71 ± 15 years and 51.6% of men had hypoglycaemia.…”

“…When the blood glucose level drops, the sympathetic nervous system cannot be effectively excited, and asymptomatic hypoglycemia is prone to occur [ 30 ]. There are also studies that use the course of diabetes as a predictor of hypoglycemia in hospitalized patients [ 31 ]. In addition, elderly patients have decreased kidney and liver metabolic function and poor drug clearance and are hence prone to hypoglycemia during intensive insulin therapy [ 32 ].…”

“…7 Events of hypoglycemia during the course of a hospitalization, even in patients with T2DM, are common. 23 In a large cohort study by Akirov and colleagues, 24 15% of in-patient PWD experienced an event of hypoglycemia, with 7% of these involving blood glucose values of <54 mg/dL (3.0 mmol/L).…”

“…Hypoglycemia episodes occur more frequently in hospitalized patients with the following characteristics: older age, cognitive dysfunction, lower body mass index, long diabetes duration, history of cerebrovascular disease, history of chronic kidney disease, A1C >9.0% (75 mmol/mol), and insulin treatment. 23,24 Hypoglycemia risk in hospitalized PWD increases by 11% with each decade of life. 24 In long-term care facilities, 25% to 35% of residents are admitted with a diagnosis of diabetes.…”
