Objective: This study's objective was to analyze the effectiveness of educational and support programs for caregivers on reducing their burden. Method: The method used was a systematic review. The following were searched; MEDLINE, LILACS, Embase, Cochrane, Web of Science, SciELO and CINAHL. Results: Seven randomized clinical studies were included. These studies compared an educational program with standard care delivery, assessing the burden of caregivers through the Zarit Burden Interview. After the analysis of sensitivity, four studies were grouped in the meta-analysis showing a statistically significant reduction in caregiver burden among the participants of educational and support programs. Conclusion: The evidence obtained in this study suggests that educational and support programs have a positive impact on the reduction of caregiver burden when compared to standard care. Therefore, the inclusion of these programs in institutions providing care to the elderly is effective and should be encouraged. These programs should also share non-pharmacological management strategies for the behavioral and psychological symptoms of dementia.Descriptors: Dementia; Caregivers; Aged; Geriatric Nursing. Método: El método utilizado fue una revisión sistemática. Consultamos el MEDLINE, LILACS, Embase, Cochrane, CINAHL y Web of Science.Resultados: Se incluyeron los ensayos controlados aleatorios que compararon el programa de educación con la atención habitual, con evaluación de la carga del cuidador a través de la Escala de Zarit Burden. Tras el análisis de sensibilidad, cuatro estudios se combinaron en un metaanálisis, que muestra una mayor reducción de la carga del cuidador entre los participantes de los programas educativos y de apoyo, con significación estadística. Conclusión: La evidencia en este estudio indica que los programas educativos y de apoyo son favorables para la reducción de la carga del cuidador. La inclusión de este tipo de programas en las instituciones dedicadas al cuidado de los ancianos es eficaz y debe fomentarse. Objectives: this study assessed burden, coping, physical symptoms and psychological morbidity in caregivers of functionally dependent family members. Methods: fifty family caregivers completed self-reported measures of burden, physical symptoms, psychological morbidity and coping strategies. Results: there was a significant negative correlation between coping strategies and the different clinical variables, as well as a significant positive correlation between coping strategies and duration of care. It appears that the stronger bond between caregiver and family member leads to a poorer use of adaptive coping strategies. It also appears that the deterioration of the relationship between them and the lower perceived self-efficacy are more prominent in caregivers of family members with cognitive impairment, indicating that caregivers with family members without cognitive impairment face fewer difficulties. Conclusion: these results emphasize the need for interventions to include coping strategies, since they are important in reducing caregivers' burden, psychological morbidity and physical symptoms.

resumoOs transtornos neuropsiquiátricos na demência, também denominados de sintomas comportamentais e psicológicos da demência (SCPD), têm prevalência elevada no curso clínico da doença de Alzheimer e de outras desordens relacionadas. A relevância dessas manifestações é bem reconhecida, estando relacionada a evolução clínica mais desfavorável, maior sobrecarga do cuidador e maior incidência de institucionalização, entre outros fatores. O objetivo desta conferência clínica é procurar responder a algumas questões relacionadas a este tema, com o intuito de oferecer aos leitores uma breve atualização sobre o assunto. Os seguintes tópicos foram selecionados para discussão: se a nomenclatura SCPD é adequada para denominar essas manifestações clínicas; se há uma forma de classificar e como diagnosticar esses sintomas; qual a sua prevalência, seu impacto no curso clínico das demências e quais os fatores de risco associados ao seu aparecimento; quais as bases fisiopatológicas conhecidas; e, finalmente, como abordar esses pacientes e também suas famílias e cuidadores, do ponto de vista terapêutico, tanto em relação a intervenções farmacológicas quanto não farmacológicas. AbstrACtNeuropsychiatric disturbances in dementia, also named behavioral and psychological symptoms of dementia (BPSD) introDuÇÃo ma boa forma de começarmos esta conferência seria questionando se a denominação sintomas comportamentais e psicológicos da demência, uma tradução direta do inglês behavioral and psychological symptoms of dementia (BPSD), poderia ser considerada uma expressão adequada para definir as manifestações neuropsiquiátricas dos transtornos demenciais. A denominação originalmente proposta por Finkel et al. (1996), a partir de uma reunião de consenso de especialistas em Neuropsiquiatria Geriátrica rea lizada na Austrália, em 1995, foi sintomas e sinais comportamentais e psicológicos da demência. Essa terminologia refere-se ao conjunto de sintomas e sinais relacionados a transtornos da percepção, do conteúdo do pensamento, do humor ou do comportamento que ocorrem freqüentemente em pacientes com síndrome demencial (Finkel et al., 1996).A introdução dessa nomenclatura na literatura científi-ca teve o inquestionável mérito de chamar a atenção de pesquisadores e clínicos para a relevância de tais manifestações como parte integrante da síndrome demencial, uma vez que estas eram menos valorizadas do que as alterações cognitivas. No entanto, pouco tempo após a publicação desse consenso, o termo "sinais" acabou sendo esquecido, o que restringiu a abrangência do conceito inicialmente proposto. Provavelmente isso se deu apenas pela maior facilidade no emprego do termo/sigla e não por qualquer motivo científico, mas passados 11 anos da publicação do consenso, a denominação original acabou sendo modificada, o que gera uma incorreção.
