Background: Epicardial fat tissue (EFT) is the visceral fat distributed along the coronary arteries between the pericardium and the myocardium. Increases in EFT are closely related to the occurrence of diabetes mellitus (DM) and cardiovascular disease. To further understand the link between EFT and DM, we conducted a meta-analysis of the relevant literature. Methods: We systematically searched electronic databases for studies on EFT performed in DM patients and published up to 30 September 2018. We included data on EFT in a DM patient group and a non-DM control group. We then assessed the effect of DM on EFT by meta-analysis and trial sequential analysis (TSA). All statistical analyses were performed using Stata 12.0 and TSA software. Results: A total of 13 studies (n = 1102 patients) were included in the final analysis. Compared with the control group, DM patients had significantly higher EFT (SMD: 1.23; 95% CI 0.98, 1.48; P = 0.000; TSA-adjusted 95% CI 0.91, 2.13; P < 0.0001). The TSA indicated that the available samples were sufficient and confirmed that firm evidence was reached. According to the regression analysis and subgroup analyses, DM typing, EFT ultrasound measurements, total cholesterol (TC) and triglyceride (TG) levels were confounding factors that significantly affected our results. Conclusions: Our meta-analysis suggests that the amount of EFT is significantly higher in DM patients than in non-DM patients.

Giriş: Atriyal fibrilasyonlu hastalarda tromboemboli riskini belirlemede en yaygın kullanılan skorlama sistemi CHA2DS2-VASc skorudur. Trombosit/lenfosit oranı (TLO) özellikle kanser hastalarında tanımlanan inflamasyonun önemli bir belirtecidir. Bizim bu çalışmamızın amacı kapağa bağlı olmayan atriyal fibrilasyonlu (KBOAF) hastalarda, TLO ile CHA2DS2-VASc skoru arasındaki ilişkiyi araştırmaktır. Hastalar ve Yöntem:Bu retrospektif çalışmada, kliniğimize 2010 ve 2016 yılları arasında başvuran 184 kapağa bağlı olmayan atriyal fibrilasyonlu hastanın (ortalama yaş 68 ± 11.4 ve % 51.6 erkek) medikal kayıtları retrospektif olarak tarandı. The most commonly used scoring system in determining the risk of thromboembolism in atrial fibrillation (AF) patients is CHA2DS2-VASc score. Platelet/lymphocyte ratio (PLR) is recently reported as an important indicator of inflammation especially in patients with malignancy. In this study, our aim was to investigate the association between PLR and CHA2DS2-VASc scores in patients with nonvalvular AF. Patients and Methods:In this retrospective study, medical records of 184 patients (Mean age 6 8 ± 11.4 years and 51.6% men) admitted to our clinic with nonvalvular AF between the years 2010 and 2016 were examined. Results:Patients with high risk of stroke according to CHA2DS2-VASc score had greater PLR, less left ventricular ejection fraction (LVEF), and less white blood cell counts compared to those of low and moderate risk groups combined. Glomerular filtration rate (GFR) values were also found lower in high risk group, albeit with a borderline statistical significance. In correlation analysis, CHA2DS2-VASc score demonstrated moderate positive correlation with PLR (r= 0.3, p< 0.01), moderate negative correlation with LVEF (r= -0.383, p< 0.01), and a weak negative correlation with GFR (r= -0.235, p= 0.01) and WBC (r= -0.235, p= 0.01). In multivariate logistic regression analysis, PLR and LVEF were shown to be independent predictors of CHA2DS2-VASc score. Area under the curve (AUC) was 0.61 (0.53-0.7, p< 0.05) for PLR and 0.69 (0.61-0.77, p< 0.001) for LVEF. The PLR value 112.5 predicted high CHA2DS2-VASc scores with a sensitivity of 65% and specifity of 61%. Conclusion:Our findings suggest PLR as a predictor of CHA2DS2-VASc scores in patients with nonvalvular AF.

The aim of this study was to investigate neutrophil-to-lymphocyte ratio (NLR), platelet-tolymphocyte ratio (PLR) and epicardial fat thickness (EFT) in diabetic and non-diabetic patients, and to determine the factors affecting EFT in diabetic patients. Materials and Methods: In this cross-sectional study, 92 diabetic patients and 40 healthy volunteers without diabetes, admitted to our outpatient clinic, were included. Demographic data, anthropometric measurements, biochemical and hematological results, echocardiography findings and EFT were recorded. NLR and PLR were calculated. These values were compared statistically between the two groups. Results: The two groups were similar in terms of age, gender, smoking status, diastolic blood pressure, total cholesterol, high-density lipoprotein (HDL), low-density lipoprotein and PLR. Body mass index (BMI), frequency of hypertension, systolic blood pressure, serum glucose, HbA1C, serum creatinine, triglyceride, total cholesterol/HDL, NLR and EFT were significantly higher in the diabetic group than the control group. Ejection fraction was significantly lower in diabetic group than the controls. NLR and BMI were independent predictors of EFT in diabetic patients. Conclusion: We suggest that echocardiographic EFT measurement, NLR and BMI might be useful in the evaluation of cardiovascular risk in diabetic patients. Amaç: Çalışmamızda diyabetik ve diyabetik olmayan hastalarda nötrofil/lenfosit oranı (NLR), platelet/lenfosit oranı (PLR) ve Eeikardiyal yağ dokusu kalınlığını (EFT) karşılaştırmayı ve diyabetik hastalarda EFT'yi etkileyen faktörleri belirlemeyi amaçladık. Gereç ve Yöntem: Bu kesitsel çalışmada polikliniğimize başvuran ve diyabeti olan 92 hasta ile birlikte, diyabeti olmayan 40 sağlıklı gönüllü çalışmaya dahil edildi. Tüm katılımcıların özgeçmişi, boy, kilo, vücut kütle indeksleri (BMI), sigara alışkanlıkları, fizik muayene bulguları, biyokimyasal ve hematolojik sonuçları ve ekokardiografi bulguları kaydedildi. Bu değerler iki grup arasında istatistiksel olarak karşılaştırıldı. Bulgular: Gruplar yaş, cinsiyet, sigara alışkanlığı, diyastolik kan basıncı, total kolesterol, high-density lipoprotein (HDL), low-density lipoprotein ve PLR açısından benzerdi. BMI, hipertansiyon, sistolik kan basıncı, serum glukoz, HbA1C, serum kreatinin, trigliserid, total kolesterol/HDL oranı, NLR ve EFT diyabetik hastalarda kontrol grubuna göre istatistiksel olarak anlamlı şekilde yüksek, ejeksiyon fraksiyonu ise anlamlı olarak düşüktü. NLR ve BMI diyabetik hastalarda EFT'nin bağımsız prediktörü olarak tespit edildi. Sonuç: Ekokardiografik EFT ölçümü, basit hematolojik parametrelerden biri olan NLR ve son olarak BMI diyabetik hastalarda kardiovasküler risk değerlendirilmesinde kullanılabilir.
