Background: Unprotected sexual activity predisposes adolescents to serious consequences later in life. This study aimed to identify the risks and protective factors associated with sexual activity in Malaysian adolescents aged 18 to 19 years. Methods: Data from a health screening program among adolescents conducted in 2010 were analyzed. A total of 21,438 unmarried adolescents responded to this topic using a self-administered non-anonymous validated questionnaire. Results: Among the participants, 54.5% were males and 45.5% were females. Ethnicities included 66.2% Malays, 20.7% Chinese, 7.2% Indians and 5.9% other ethnicities. Most of the respondents (97.1%) reported having a secondary level of education. The overall prevalence of sexual activity in older adolescents was 6.4% (8.9% among males and 3.6% among females). In a multivariate logistic regression model, sexual activity was positively associated with pornographic viewing (adjusted odds ratio [aOR]: 2.55; 95% confidence interval [CI]: 2.01-3.22), risky behavior (aOR: 2.62; 95%CI: 2.11-3.25), anti-social behavior (aOR: 2.44; 95%CI: 2.06-2.90), female (aOR: 2.24; 95%CI: 1.76-2.85), history of abuse (aOR: 1.81; 95%CI: 1.46-2.24), low religiosity (aOR: 1.71; 95%CI: 1.30-2.26) and masturbation (aOR: 1.34; 95%CI: 1.07-1.68). Conclusions: A comprehensive intervention program should be developed to target this at-risk group to prevent the spread of sexually transmitted diseases and criminal abortion.

IntroduçãoA primeira relação sexual, considerada um marco na vida dos jovens, tem iniciado cada vez mais precocemente. No contexto brasileiro, a idade média da primeira relação sexual é de 14 anos para o sexo masculino e 15 para o feminino 1 .As mulheres priorizam o sentimento de "entrega e amor" na primeira relação sexual, ao mesmo tempo em que existe o desejo de se descobrir, impõe-se a necessidade de se "preservar". Em contrapartida, a experiência sexual masculina é vista como um ganho, sustentando o poder da masculinidade 2 . Estudo revela que jovens tendem a não usar preservativo no início de sua vida sexual e definem esta relação como casual. Os principais motivos alegados para a sua não utilização de modo consistente são: não gostar de usá-los, confiar no parceiro e a imprevisibilidade das relações sexuais 3 .Além disso, há uma relação entre o uso de ál-cool e outras substâncias psicoativas com comportamentos sexuais de risco na adolescência 4 . Ademais, investigações indicam que a iniciação sexual precoce está associada a um maior risco de contaminação por doenças sexualmente transmissíveis (DST) e gravidez na adolescência 5,6,7 . A literatura científica sugere que os fatores associados à iniciação sexual dependem de aspectos culturais. Family environment is one of the most influential factors on youth sexual behavior but has received little investigation in sub-Saharan Africa. Yet, a comprehensive understanding of family influences could improve the efficiency of reproductive health (RH) interventions. Using retrospective data from a population-based survey, life-table analysis was utilized to compute the median age at premarital intercourse among 1,182 youth aged 12-24 years. Discrete-time hazard models were used in multivariate analysis to estimate the effects of family structures and parent-child interactions on premarital intercourse. Overall, 42% of participants had a premarital intercourse, and the median age at first sex was 16.9 years. Participants in nuclear two-parent families had the highest median (17.7) compared with those in nuclear one-(16.7), extended one-(16.5), extended two-parent families (16.9) or other relatives (16.8). Youth from monogamous families had the highest median (16.9) compared to those from polygamous (16.3) and other families (16.0). Orphans and youth reporting family transitions were more likely to initiate first sex at an earlier age compared with non-orphans and non-movers, with a median of 16.1 and 16.9 years, respectively. Multivariate results showed that living in extended families, being orphaned, and family transitions significantly increased the risk of premarital intercourse. Polygamy showed marginal effects. Stronger parent-child relationships and higher levels of parental control decreased the risk of premarital intercourse. Unexpectedly, parent-child communication was significantly associated with a higher risk of sexual debut. Programmatically, family environment is an important resource that needs to be promoted when designing RH interventions in sub-Saharan Africa.
