ObjectiveSexual dysfunction in multiple sclerosis (MS) is a significant, but often underestimated and overlooked suffering. Interventions to treat sexual dysfunction in MS are rare. The relation between sexual dysfunction in MS and psychological as well as neuropsychological aspects is evident. However, this field of research remains markedly underdeveloped in this severe chronic illness. The aim of this scoping review is to describe the relevant knowledge in this area and to identify psychological interventions to treat sexual dysfunctions in MS.MethodsA scoping review was conducted to answer the following questions: (1) Which psychological and neuropsychological factors impact on sexual dysfunction in MS and vice versa? (2) What kind of psychological interventions aiming to improve sexual dysfunctions in MS are available? A comprehensive search and review of MEDLINE, PsycINFO, and CINAHL was completed by using a recent methodological framework for scoping reviews.Results23 publications covering a total of 13,259 people with MS and 532 healthy controls were identified. Sexual dysfunction was found to be very common in MS and there is an obvious relation to psychological disorders as e.g. depression and anxiety and also to psychological aspects as partner relationship and quality of life. The relation between sexual dysfunction in MS and neuropsychological impairment has only rarely been studied and no clear results were found. Only two studies were identified, assessing the effectiveness of psychological intervention studies on sexual dysfunction in people with MS, and a third study presenting a secondary analysis of a study targeting depression. All three studies reported significant improvements in sexual dysfunction as well as partly in psychological variables.ConclusionsThere is a pressing need for the development and adequate evaluation of psychological interventions for sexual dysfunctions in MS. In addition, sexual dysfunction and its impact on psychological wellbeing should be more focussed in clinical care.RegistrationThis review is registered with PROSPERO; Registration number: CRD42016033066.

Multiple sclerosis (MS) is a common inflammatory disorder of the central nervous system (CNS) in which the myelin sheath is targeted and subsequently degraded. In MS, disease modifying therapies (DMT) aim to decrease the relapse rates and decrease the number of lesions in the brain and spinal cord. Several different types of DMTs exist with distinct properties and mechanisms of action. This is beneficial because the clinical course of MS varies from individual to individual, and so treatment must be tailored to each individual's unique disease.Since the proportion of women with MS is larger than that of men, the concept of managing MS with DMT while also taking into account pregnancy and fertility issues constitutes an important discussion. Disease modifying therapy (DMT) aims to reduce relapse rates and decrease the quantity of lesions in the brain and spinal cord. Since MS is more prevalent in women than men, it is important to be aware of the interplay between MS and pregnancy. As MS can engender sexual dysfunction, primarily in the form of decreased desire and fatigue, it thereby affects conception. Hormonal differences between women with MS compared to women without MS include an increase in follicle-stimulating hormone and luteinizing hormone, and a decrease in testosterone. While fluctuations in estrogen result in a reduction in MS relapse rates during pregnancy, a subsequent increase in the post-partum period is observed. The mechanism of action and side effects of DMTs are described in this paper, including interferon, glatiramer acetate, and some newer medications. Although there are no recommended guidelines on the use of DMTs during pregnancy, it is generally agreed upon to cease their use prior to conception if possible, and the decision to continue a DMT should take into account the benefits to the mother and the risks to the fetus. The brain avatar of schizophrenic patients is different from the normal human brain avatar, and it is difficult to overcome the complex environmental effects of the brain through traditional magnetic resonance imaging (MRI). In order to improve the accuracy of MRI in detecting brain information in patients with schizophrenia, this study is based on the support vector machine classification algorithm and combined with multimodal MRI detection method to construct a detection model suitable for patients with schizophrenia. In addition, this study combines the existing test cases to divide the brain into regions and design a comparative experiment to study the accuracy of the model proposed in this study. Finally, the study draws the results by sub‐regional comparison. Studies have shown that the algorithm model of this study has certain effects on brain detection in patients with schizophrenia, and can be applied to practice, and can provide theoretical reference for subsequent related research.
