Background: Intra cytoplasmic sperm injection (ICSI) is one of the effective treatments in women with fertility problems; however, there are no approved predictors for pregnancy till the present moment. We aimed to evaluate the role of progesterone and endometrium pattern in predicting pregnancy in ICSI cycles. Patients and Methods: The study included 200 women who were treated in Al-Shatby Hospital in (ICSI) Unit from January 2017 to October 2018. They underwent one super ovulation protocol which was gonadotrophin in which releasing hormone progesterone was measured in the 14 th day; while ultrasound was performed in the 3rd day and in the day of HCG administration. Results and Conclusion: Progesterone with a cutoff point of 27 ng/mol is a significant predictor of pregnancy in ICSI cycle. However, neither endometrial thickness nor pattern was a significant predictor of pregnancy.

Background: With advances in assisted reproductive techniques its becoming increasingly important to identify pregnancies having a potential of adverse outcome in the form of per vaginal bleeding or early pregnancy loss. The objective of this study was to find out whether the value of luteal phase progesterone can be used as a marker to predict the possibility of per vaginal bleeding and early pregnancy loss in cases conceived by in vitro fertilization (IVF) and embryo transfer (ET).Methods: A total of 40 women of age less than or equal to 40 years undergoing IVF and ET were included in this study depending upon inclusion criteria. Patients were excluded if they had any factor defined as exclusion criteria. on D14 after embryo transfer B-hCG was done in all the patients. If B-hCG levels were found to be ≥ 100 mIU/ml then serum progesterone levels were also done. B-hCG levels were repeated after 48 hours. Pregnancies were followed up and correlation between suboptimal rise in serum progesterone levels and adverse pregnancy outcome was studied.Results: Mean Beta-hCG and Serum progesterone levels on D14 of embryo transfer were found to be 388.86±34 mIU/ml and 54.24±4.32 ng/ml respectively. A repeat B-hCG and serum progesterone level 48 hours after initial estimation showed mean B-hCG and serum progesterone level to be 598.80±52.12 mIU/ml and 72. 24±5.24 ng/ml respectively. Out of 40 patients 26 patients showed >30% rise in serum progesterone level while 14 patients showed less than 30% rise in serum progesterone level.Conclusions: Suboptimal rise in serum progesterone level (<30%) was associated with increased incidence of adverse pregnancy outcome in women conceived after in vitro fertilization and embryo transfer.

Human chorionic gonadotrophin (hCG) is the first specific molecule synthesized by the embryo. hCG RNA is transcribed as early as the eight-cell stage, and the blastocyst produces the protein before its implantation. hCG in the uterine microenvironment binds with its cognate receptor, luteinizing hormone/choriogonadotropin receptor (LHCGR), on the endometrial surface. This binding stimulates leukemia inhibitory factor (LIF) production and inhibits interleukin-6 (IL-6) production by epithelial cells of the endometrium. These effects ensure essential help in the preparation of the endometrium for initial embryo implantation. hCG also effects angiogenic and immunomodulatory actions as reported in many articles by our laboratories and other ones. By stimulating angiogenesis and vasculogenesis, hCG provides the placenta with an adequate maternal blood supply and optimal embryo nutrition during the invasion of the uterine endometrium. The immunomodulatory properties of hCG are numerous and important for programming maternal immune tolerance toward the embryo. The reported effects of hCG on uterine NK, Treg, and B cells, three major cell populations for the maintenance of pregnancy, demonstrate the role of this embryonic signal as a crucial immune regulator in the course of pregnancy. Human embryo rejection for hCG-related immunological reasons has been studied in different ways, and a sufficient dose of hCG seems to be necessary to maintain maternal tolerance. Different teams have studied the addition of hCG in patients suffering from recurrent miscarriages or implantation failures. hCG could also have a beneficial or a negative impact on autoimmune diseases during pregnancy. In this review, we will discuss the immunological impacts of hCG during pregnancy and if this hormone might be used therapeutically.
