Benign prostatic hyperplasia (BPH) and prostate cancer (PCa) share common conditions such as lower urinary tract symptoms (LUTS) and dyslipidaemia. Whether an extensive lipid profile analysis could discriminate between BPH and PCa was the objective. Thirty‐six (36) BPH and twenty (20) PCa outpatients of a urology clinic plus forty (40) controls without LUTS, but normal PSA, were recruited. Body mass index (BMI), lipid profile (total cholesterol [CHOL], triglycerides [TG], high‐density lipoprotein [HDL], very‐low‐density lipoprotein [VLDL], low‐density lipoprotein [LDL] and Castelli's risk index I [CR I] [TC/HDL]), oxidised LDL, apolipoprotein E, ceramide and PSA were determined. Mean ages for BPH, PCa and control were 69 ± 13, 67 ± 10 and 53 ± 7 years respectively. Most parameters apart from BMI and HDL were significantly different compared to the control group. oxLDL for BPH versus control, PCa versus control and BPH versus PCa was significant (p < 0.001, p = 0.02 and p < 0.001 respectively). Ceramide showed significant group differences. Between BPH and PCa, total cholesterol, LDL and Apo E were significantly different (p = 0.00, p = 0.01 and p = 0.03 respectively). Apo E could potentially be a discriminating biomarker. Receiver operating characteristic curves for TPSA, Apo E and oxLDL demonstrated sensitivity of 69.44 and specificity of 88.24 for oxLDL, hence more discriminatory.

Screening the active compounds of herbal medicines is of importance to modern drug discovery. In this work, an integrative strategy was established to discover the effective compounds and their therapeutic targets using Phellodendri Amurensis cortex (PAC) aimed at inhibiting prostate cancer as a case study. We found that PAC could be inhibited the growth of xenograft tumours of prostate cancer. Global constituents and serum metabolites were analysed by UPLC-MS based on the established chinmedomics analysis method, a total of 54 peaks in the spectrum of PAC were characterised in vitro and 38 peaks were characterised in vivo. Among the 38 compounds characterised in vivo, 29 prototype components were absorbed in serum and nine metabolites were identified in vivo. Thirty-four metabolic biomarkers were related to prostate cancer, and PAC could observably reverse these metabolic biomarkers to their normal level and regulate the disturbed metabolic profile to a healthy state. A chinmedomics approach showed that ten absorbed constituents, as effective compounds, were associated with the therapeutic effect of PAC. In combination with bioactivity assays, the action targets were also predicted and discovered. As an illustrative case study, the strategy was successfully applied to high-throughput screening of active compounds from herbal medicine.

Background Prostate cancer (PCa) is the second most common cancer among men worldwide. Recent research has identified [−2]proPSA (p2PSA), %p2PSA and prostate health index ( phi ) as new biomarkers for the early diagnosis and grading of PCa. However, few studies have used these parameters in a healthy population. In this study, we aimed to establish reference intervals (RIs) for p2PSA, %p2PSA and phi in healthy men based on age stratification. Methods Between April 2016 and March 2018, healthy subjects were recruited. Healthy men were then stratified into four age groups: <40 years, 40–49 years, 50–59 years and ≥60 years. Total PSA (tPSA), free PSA (fPSA), %fPSA, p2PSA, %p2PSA and phi were measured and RIs were established for p2PSA, %p2PSA and phi . Results In total, 732 healthy men were used for analysis. The RIs of phi were 9.77–48.44 for <40 years of age, 9.85–65.28 for 40–49 years of age, 9.98–39.72 for 50–59 years of age and 8.16–40.76 for ≥60 years of age. The reference values at the age of 40–49 years were generally higher than those at ≥60 years of age. Conclusions Age-specific RIs for p2PSA, %p2PSA and phi were established in this study. This first set of established RIs will be invaluable for physicians to make precise medical decisions and carry out appropriate medical interventions.
