Predictive markers, including total lesion glycolysis, for the response of lymph node(s) metastasis from head and neck squamous cell carcinoma treated by chemoradiotherapy

Abstract: TLG was found to be a good predictive factor for metastatic lymph node(s) prior to CRT treatment. After CRT treatment, FDG-PET was found to be highly specific and useful for negative screening.

“…We found a strong correlation between MLV and SUV mean (r = 0.90); this result is probably due to the few patients included in the study. Our result is consistent with observations in oncology, with some studies suggesting TLG as a better predictor of outcomes in patients with lung or cervical cancer [17, 25]. We found MLV independently associated with prognosis, like TLG, so the volume of the lung with an altered uptake may be more informative to assess prognosis than the mean SUV in this volume.…”

“…Therefore, the concept of metabolic tumor burden, such as MTV and TLG, has been introduced to consider both FDG uptake and tumor volume. In this study, MTV 2.5 and TLG 2.5 differed significantly with respect to PFS and OS, like the results of previous studies . In addition, we examined whether the changing rate of MTV and TLG, which were not used in those studies, can predict patient survival.…”

“…Cervical lymph node metastases were defined using the criteria reported by Nakamura et al The maximum standardized uptake value (SUV max ), MTV, and TLG were calculated for all lesions for both the early and delayed phases. The cutoff SUVs for calculating MTV and TLG were defined as 2.5% and 40% of the SUV max , respectively, based on the results of previous reports . MTV was defined as the summed volume of the voxels whose SUV was higher than the cutoff value in semiautomatically delineated VOI.…”

“…Two patient groups were established according to the results of the PET/MR therapeutic response evaluation and the clinical follow-up. Furthermore, patient subgroups were also set up, namely a complete remission (CR) group defined as patients with an absence of a viable primary tumor tissue and a non-complete remission (NCR) group defined as patients with any pernicious proliferations including partial response, stable disease, and progressive disease groups ( 24 , 25 ).…”
